Actually we only process title and description from exercise and questions,
but not process answers.
Process answers is posponed until find a good point where to retrieve both
question and answers data. We must treat this tool in a special way to avoid
re-indexing at one move.

- question.class.php is now an abstract class. hotspot.class.php, uniquer_answer.class.php are extending Question.
- when we create / edit a question, the question form and the answers form are now on the same page (except for hotspot)
