Patch that makes quoting "sameuser", "samegroup", and "all" remove

special meaning of these terms in pg_hba.conf.

Also changes ugly pg_hba.conf IPv6 netmask of
ffff:ffff:ffff:ffff:ffff:ffff:ffff:ffff to ::1/128.
