Convert assorted internal-ish datatypes, namely aclitemin,
int2vectorin, oidin, oidvectorin, pg_lsn_in, pg_snapshot_in,
and tidin to the new style.
(Some others you might expect to find in this group, such as
cidin and xidin, need no changes because they never throw
errors at all. That seems a little cheesy ... but it is not in
the charter of this patch series to add new error conditions.)

This code seems to have been written on the assumption that
"unsigned long" is 32 bits; or at any rate it ignored the
possibility of conversion overflow. Rewrite, borrowing some
logic from oidin().

currtid() and currtid2() are an undocumented set of functions whose sole
known user is the Postgres ODBC driver, able to retrieve the latest TID
version for a tuple given by the caller of those functions.
As used by Postgres ODBC, currtid() is a shortcut able to retrieve the
last TID loaded into a backend by passing an OID of 0 (magic value)
after a tuple insertion. This is removed in this commit, as it became
obsolete after the driver began using "RETURNING ctid" with inserts, a
clause supported since Postgres 8.2 (using RETURNING is better for
performance anyway as it reduces the number of round-trips to the
backend).
currtid2() is still used by the driver, so this remains around for now.
Note that this function is kept in its original shape for backward
compatibility reasons.

uses RELCACHE_FORCE_RELEASE via the tests introduced recently in

A relation that has no storage initializes rd_tableam to NULL, which
caused those two functions to crash because of a pointer dereference.
Note that in 11 and older versions, this has always failed with a
confusing error "could not open file".
These two functions are used by the Postgres ODBC driver, which requires
them only when connecting to a backend strictly older than 8.1. When
connected to 8.2 or a newer version, the driver uses a RETURNING clause
instead whose support has been added in 8.2, so it should be possible to
just remove both functions in the future. This is left as an issue to
address later.
While on it, add more regression tests for those functions as we never
really had coverage for them, and for aggregates of TIDs.

This also involves renaming src/include/utils/hashutils.h, which
becomes src/include/common/hashfn.h. Perhaps an argument can be
made for keeping the hashutils.h name, but it seemed more
consistent to make it match the name of the file, and also more
descriptive of what is actually going on here.

Commit 147e3722f7 changed Tid scan so that it calls table_beginscan()
and uses the scan option for seq scan. This change caused two issues.
(1) The change caused Tid scan to take a predicate lock on the entire
relation in serializable transaction even when relation-level
lock is not necessary. This could lead to an unexpected
serialization error.
(2) The change caused Tid scan to increment the number of seq_scan
in pg_stat_*_tables views even though it's not seq scan. This
could confuse the users.
This commit adds the scan option for Tid scan and makes Tid scan
use it, to avoid those issues.
Back-patch to v12, where the bug was introduced.

Some of the wrapper functions didn't match the callback names. Many of
them due to staying "consistent" with historic naming of the wrapped
functionality. We decided that for most cases it's more important to
be for tableam to be consistent going forward, than with the past.
The one exception is beginscan/endscan/... because it'd have looked
odd to have systable_beginscan/endscan/... with a different naming
scheme, and changing the systable_* APIs would have caused way too
much churn (including breaking a lot of external users).

Instead add a tableam callback to do so. To avoid adding per
validation overhead, pass a scan to tuple_tid_valid. In heap's case
we'd otherwise incurred a RelationGetNumberOfBlocks() call for each
tid - which'd have added noticable overhead to nodeTidscan.c.

This primarily is to allow WHERE CURRENT OF to continue to work as it
currently does. It's not clear to me that these semantics make sense
for every AM, but it works for the in-core heap, and the out of core
zheap. We can refine it further at a later point if necessary.

access/hash is the place for the hash index AM, not really appropriate
for generic facilities, which is what hash_any is; having things the old
way meant that anything using hash_any had to include the AM's include
file, pointlessly polluting its namespace with unrelated, unnecessary
cruft.
Also move the HTEqual strategy number to access/stratnum.h from
access/hash.h.
To avoid breaking third-party extension code, add an #include
"utils/hashutils.h" to access/hash.h. (An easily removed line by
committers who enjoy their asbestos suits to protect them from angry
extension authors.)

Up to now we've not worried much about joins where the join key is a
relation's CTID column, reasoning that storing a table's CTIDs in some
other table would be pretty useless. However, there are use-cases for
this sort of query involving self-joins, so that argument doesn't really
hold water.
With larger relations, a merge or hash join is desirable. We had a btree
opclass for type "tid", allowing merge joins on CTID, but no hash opclass
so that hash joins weren't possible. Add the missing infrastructure.
This also potentially enables hash aggregation on "tid", though the
use-cases for that aren't too clear.

AclObjectKind was basically just another enumeration for object types,
and we already have a preferred one for that. It's only used in
aclcheck_error. By using ObjectType instead, we can also give some more
precise error messages, for example "index" instead of "relation".

This is a mechanical change in preparation for a later commit that
will change the layout of TupleDesc. Introducing a macro to abstract
the details of where attributes are stored will allow us to change
that in separate step and revise it in future.

There are no functional changes here; this simply encapsulates knowledge
of the ItemPointerData struct so that a future patch can change things
without more breakage.
All direct users of ip_blkid and ip_posid are changed to use existing
macros ItemPointerGetBlockNumber and ItemPointerGetOffsetNumber
respectively. For callers where that's inappropriate (because they

This makes almost all core code follow the policy introduced in the
previous commit. Specific decisions:
- Text search support functions with char* and length arguments, such as
prsstart and lexize, may receive unaligned strings. I doubt
maintainers of non-core text search code will notice.
- Use plain VARDATA() on values detoasted or synthesized earlier in the
same function. Use VARDATA_ANY() on varlenas sourced outside the
function, even if they happen to always have four-byte headers. As an
exception, retain the universal practice of using VARDATA() on return
values of SendFunctionCall().
- Retain PG_GETARG_BYTEA_P() in pageinspect. (Page images are too large
for a one-byte header, so this misses no optimization.) Sites that do
not call get_page_from_raw() typically need the four-byte alignment.
- For now, do not change btree_gist. Its use of four-byte headers in
memory is partly entangled with storage of 4-byte headers inside
GBT_VARKEY, on disk.
- For now, do not change gtrgm_consistent() or gtrgm_distance(). They
incorporate the varlena header into a cache, and there are multiple
credible implementation strategies to consider.
This avoids additional translatable strings for each distinct type, as
well as making our quoting style around type names more consistent
(namely, that we don't quote type names). This continues what started
as f402b99501.

This has a slight performance cost, but the only known consumers
of these functions, known at the SQL level as currtid and currtid2,
is pgsql-odbc; whose usage, we hope, is not sufficiently intensive
to make this a problem.

This commit changes index-only scans so that data is read directly from the
index tuple without first generating a faux heap tuple. The only immediate
benefit is that indexes on system columns (such as OID) can be used in
index-only scans, but this is necessary infrastructure if we are ever to
support index-only scans on expression indexes. The executor is now ready
for that, though the planner still needs substantial work to recognize
the possibility.
To do this, Vars in index-only plan nodes have to refer to index columns
not heap columns. I introduced a new special varno, INDEX_VAR, to mark
such Vars to avoid confusion. (In passing, this commit renames the two
existing special varnos to OUTER_VAR and INNER_VAR.) This allows
ruleutils.c to handle them with logic similar to what we use for subplan
reference Vars.
Since index-only scans are now fundamentally different from regular
indexscans so far as their expression subtrees are concerned, I also chose
to change them to have their own plan node type (and hence, their own
executor source file).

certain highly-loaded headers such as rel.h and relscan.h, instead using just
relcache.h, heapam.h or genam.h, which are more lightweight and thus cause less
unnecessary dependencies.

For this to work, a new header file access/sysattr.h needed to be created,
initially containing attribute numbers of system columns, for pg_dump usage.
While at it, make contrib ltree, intarray and hstore header files more
consistent with our header style.
While it's not clear that TID linkage info is of any great use to a
nefarious user, it's certainly unexpected that these functions wouldn't
insist on read privileges.
opclass. This is not so much because anyone's likely to create an index
on TID, as that sorting TIDs can be useful. Also added max and min
aggregates while at it, so that one can investigate the clusteredness of
a table with queries like SELECT min(ctid), max(ctid) FROM tab WHERE ...
