The code in charge of processing a single invalidation message has been
using since 568d413 the structure for relation mapping messages. This
had fortunately no consequence as both locate the database ID at the
same location, but it could become a problem in the future if this area
of the code changes.

When wal_level=logical, write invalidations at command end into WAL so
that decoding can use this information.
This patch is required to allow the streaming of in-progress transactions
in logical decoding. The actual work to allow streaming will be committed
as a separate patch.
We still add the invalidations to the cache and write them to WAL at
commit time in RecordTransactionCommit(). This uses the existing
XLOG_INVALIDATIONS xlog record type, from the RM_STANDBY_ID resource
manager (see LogStandbyInvalidations for details).
So existing code relying on those invalidations (e.g. redo) does not need
to be changed.
The invalidations written at command end uses a new xlog record type
XLOG_XACT_INVALIDATIONS, from RM_XACT_ID resource manager. See
LogLogicalInvalidations for details.
These new xlog records are ignored by existing redo procedures, which
still rely on the invalidations written to commit records.
The invalidations are decoded and accumulated in top-transaction, and then
executed during replay. This obviates the need to decode the
invalidations as part of a commit record.
Bump XLOG_PAGE_MAGIC, since this introduces XLOG_XACT_INVALIDATIONS.

Given these routines are heap specific, and that there will be more
generic visibility support in via table AM, it makes sense to move the
prototypes to heapam.h (routines like HeapTupleSatisfiesVacuum will
not be exposed in a generic fashion, because they are too storage
specific).
Similarly, the code in tqual.c is specific to heap, so moving it into
access/heap/ makes sense.

Back in commit 100340e2dc, we made relcache entries keep lists of the
foreign keys applying to the relation -- but we forgot to update
CacheInvalidateHeapTuple to flush those entries when new FKs got created
or existing ones updated/deleted. No bugs appear to have been reported
that would be explained by this ommission, but I noticed the problem
while working on an unrelated bugfix which clearly showed it. Fix by
adding relcache flush on relevant foreign key changes.
Backpatch to 9.6, like the aforementioned commit.

Previously tables declared WITH OIDS, including a significant fraction
of the catalog tables, stored the oid column not as a normal column,
but as part of the tuple header.
This special column was not shown by default, which was somewhat odd,
as it's often (consider e.g. pg_class.oid) one of the more important
parts of a row. Neither pg_dump nor COPY included the contents of the
oid column by default.
The fact that the oid column was not an ordinary column necessitated a
significant amount of special case code to support oid columns. That
already was painful for the existing, but upcoming work aiming to make
table storage pluggable, would have required expanding and duplicating
that "specialness" significantly.
WITH OIDS has been deprecated since 2005 (commit ff02d0a05280e0).
Remove it.
Removing includes:
- CREATE TABLE and ALTER TABLE syntax for declaring the table to be
WITH OIDS has been removed (WITH (oids[ = true]) will error out)
- pg_dump does not support dumping tables declared WITH OIDS and will
issue a warning when dumping one (and ignore the oid column).
- restoring an pg_dump archive with pg_restore will warn when
restoring a table with oid contents (and ignore the oid column)
- COPY will refuse to load binary dump that includes oids.
- pg_upgrade will error out when encountering tables declared WITH
OIDS, they have to be altered to remove the oid column first.
- Functionality to access the oid of the last inserted row (like
plpgsql's RESULT_OID, spi's SPI_lastoid, ...) has been removed.
The syntax for declaring a table WITHOUT OIDS (or WITH (oids = false)
for CREATE TABLE) is still supported. While that requires a bit of
support code, it seems unnecessary to break applications / dumps that
do not use oids, and are explicit about not using them.
The biggest user of WITH OID columns was postgres' catalog. This
commit changes all 'magic' oid columns to be columns that are normally
declared and stored. To reduce unnecessary query breakage all the
newly added columns are still named 'oid', even if a table's column
naming scheme would indicate 'reloid' or such. This obviously
requires adapting a lot code, mostly replacing oid access via
HeapTupleGetOid() with access to the underlying Form_pg_*->oid column.
The bootstrap process now assigns oids for all oid columns in
genbki.pl that do not have an explicit value (starting at the largest
oid previously used), only oids assigned later by oids will be above
FirstBootstrapObjectId. As the oid column now is a normal column the
special bootstrap syntax for oids has been removed.
Oids are not automatically assigned during insertion anymore, all
backend code explicitly assigns oids with GetNewOidWithIndex(). For
the rare case that insertions into the catalog via SQL are called for
the new pg_nextoid() function can be used (which only works on catalog
tables).
The fact that oid columns on system tables are now normal columns
means that they will be included in the set of columns expanded
by * (i.e. SELECT * FROM pg_class will now include the table's oid,
previously it did not). It'd not technically be hard to hide oid
column by default, but that'd mean confusing behavior would either
have to be carried forward forever, or it'd cause breakage down the
line.
While it's not unlikely that further adjustments are needed, the
scope/invasiveness of the patch makes it worthwhile to get merge this
now. It's painful to maintain externally, too complicated to commit
after the code code freeze, and a dependency of a number of other
patches.

It's somewhat surprising that we got away with this before. (Actually,
since nobody tests this routinely AFAIK, it might've been broken for
awhile. But it's definitely broken in the wake of commit f868a8143.)
It seems sufficient to limit the forced recursion to a small number
of levels.

When vacuum processes a relation it uses the corresponding relcache
entry's relfrozenxid / relminmxid as a cutoff for when to remove
tuples etc. Unfortunately for nailed relations (i.e. critical system
catalogs) bugs could frequently lead to the corresponding relcache
entry being stale.
This set of bugs could cause actual data corruption as vacuum would
potentially not remove the correct row versions, potentially reviving
them at a later point. After 699bf7d05c some corruptions in this vein
were prevented, but the additional error checks could also trigger
spuriously. Examples of such errors are:
ERROR: found xmin ... from before relfrozenxid ...
and
ERROR: found multixact ... from before relminmxid ...
To be caused by this bug the errors have to occur on system catalog
tables.
The two bugs are:
1) Invalidations for nailed relations were ignored, based on the
theory that the relcache entry for such tables doesn't
change. Which is largely true, except for fields like relfrozenxid
etc. This means that changes to relations vacuumed in other
sessions weren't picked up by already existing sessions. Luckily
autovacuum doesn't have particularly longrunning sessions.
2) For shared *and* nailed relations, the shared relcache init file
was never invalidated while running. That means that for such
tables (e.g. pg_authid, pg_database) it's not just already existing
sessions that are affected, but even new connections are as well.
That explains why the reports usually were about pg_authid et. al.
To fix 1), revalidate the rd_rel portion of a relcache entry when
invalid. This implies a bit of extra complexity to deal with
bootstrapping, but it's not too bad. The fix for 2) is simpler,
simply always remove both the shared and local init files.

It is equivalent in ANSI C to write (*funcptr) () and funcptr(). These
two styles have been applied inconsistently. After discussion, we'll
use the more verbose style for plain function pointer variables, to make
it clear that it's a variable, and the shorter style when the function
pointer is in a struct (s.func() or s->func()), because then it's clear
that it's not a plain function name, and otherwise the excessive
punctuation makes some of those invocations hard to read.

Don't move parenthesized lines to the left, even if that means they
flow past the right margin.
By default, BSD indent lines up statement continuation lines that are
within parentheses so that they start just to the right of the preceding
left parenthesis. However, traditionally, if that resulted in the
continuation line extending to the right of the desired right margin,
then indent would push it left just far enough to not overrun the margin,
if it could do so without making the continuation line start to the left of
the current statement indent. That makes for a weird mix of indentations
unless one has been completely rigid about never violating the 80-column
limit.
This behavior has been pretty universally panned by Postgres developers.
Hence, disable it with indent's new -lpl switch, so that parenthesized
lines are always lined up with the preceding left paren.
This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

Change pg_bsd_indent to follow upstream rules for placement of comments
to the right of code, and remove pgindent hack that caused comments
following #endif to not obey the general rule.
Commit e3860ffa4d wasn't actually using
the published version of pg_bsd_indent, but a hacked-up version that
tried to minimize the amount of movement of comments to the right of
code. The situation of interest is where such a comment has to be
moved to the right of its default placement at column 33 because there's
code there. BSD indent has always moved right in units of tab stops
in such cases --- but in the previous incarnation, indent was working
in 8-space tab stops, while now it knows we use 4-space tabs. So the
net result is that in about half the cases, such comments are placed
one tab stop left of before. This is better all around: it leaves
more room on the line for comment text, and it means that in such
cases the comment uniformly starts at the next 4-space tab stop after
the code, rather than sometimes one and sometimes two tabs after.
Also, ensure that comments following #endif are indented the same
as comments following other preprocessor commands such as #else.
That inconsistency turns out to have been self-inflicted damage
from a poorly-thought-through post-indent "fixup" in pgindent.
This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

The new indent version includes numerous fixes thanks to Piotr Stefaniak.
The main changes visible in this commit are:
* Nicer formatting of function-pointer declarations.
* No longer unexpectedly removes spaces in expressions using casts,
sizeof, or offsetof.
* No longer wants to add a space in "struct structname *varname", as
well as some similar cases for const- or volatile-qualified pointers.
* Declarations using PG_USED_FOR_ASSERTS_ONLY are formatted more nicely.
* Fixes bug where comments following declarations were sometimes placed
with no space separating them from the code.
* Fixes some odd decisions for comments following case labels.
* Fixes some cases where comments following code were indented to less
than the expected column 33.
On the less good side, it now tends to put more whitespace around typedef
names that are not listed in typedefs.list. This might encourage us to
put more effort into typedef name collection; it's not really a bug in
indent itself.
There are more changes coming after this round, having to do with comment
indentation and alignment of lines appearing within parentheses. I wanted
to limit the size of the diffs to something that could be reviewed without
one's eyes completely glazing over, so it seemed better to split up the
changes as much as practical.

We have now grown enough registerable syscache-invalidation callback
functions that the original assumption that there would be few of them
is causing performance problems. In particular, let's fix things so that
CallSyscacheCallbacks doesn't have to search the whole array to find
which callback(s) to invoke for a given cache ID. Preserve the original
behavior that callbacks are called in order of registration, just in
case there's someplace that depends on that (which I doubt).
In support of this, export the number of syscaches from syscache.h.
People could have found that out anyway from the enum, but adding a
#define makes that much safer.
This provides a useful additional speedup in Mathieu Fenniak's
logical-decoding test case, although we're reaching the point of
diminishing returns there. I think any further improvement will have
to come from reducing the number of cache invalidations that are
triggered in the first place. Still, we can hope that this change
gives some incremental benefit for all invalidation scenarios.

A test case provided by Mathieu Fenniak shows that the initial search for
the target catcache in CatalogCacheIdInvalidate consumes a very significant
amount of overhead in cases where cache invalidation is triggered but has
little useful work to do. There is no good reason for that search to exist
at all, as the index array maintained by syscache.c allows direct lookup of
the catcache from its ID. We just need a frontend function in syscache.c,
matching the division of labor for most other cache-accessing operations.
While there's more that can be done in this area, this patch alone reduces
the runtime of Mathieu's example by 2X. We can hope that it offers some
useful benefit in other cases too, although usually cache invalidation
overhead is not such a striking fraction of the total runtime.
Back-patch to 9.4 where logical decoding was introduced. It might be
worth going further back, but presently the only case we know of where
cache invalidation is really a significant burden is in logical decoding.
Also, older branches have fewer catcaches, reducing the possible benefit.
(Note: although this nominally changes catcache's API, we have always
documented CatalogCacheIdInvalidate as a private function, so I would
have little sympathy for an external module calling it directly. So
backpatching should be fine.)

Increase from the historical value of 32 to 64. We are up to 31 callers
of CacheRegisterSyscacheCallback() in HEAD, so if they were all to be
exercised in one process that would leave only one slot for add-on modules.
It's probably not possible for that to happen, but still we clearly need
more daylight here. (At some point it might be worth making the array
dynamically resizable; but since we've never heard a complaint of "out of
syscache_callback_list slots" happening in the field, I doubt it's worth
it yet.)
Back-patch as far as 9.4, which is where we increased the companion limit
MAX_RELCACHE_CALLBACKS (cf commit f01d1ae3a). It's not as urgent in
released branches, which have only a couple dozen call sites in core, but
it still seems that somebody might hit the limit before these branches die.

Twiddle the replication-related code so that its timestamp variables
are declared TimestampTz, rather than the uninformative "int64" that
was previously used for meant-to-be-always-integer timestamps.
This resolves the int64-vs-TimestampTz declaration inconsistencies
introduced by commit 7c030783a, though in the opposite direction to
what was originally suggested.
This required including datatype/timestamp.h in a couple more places
than before. I decided it would be a good idea to slim down that
header by not having it pull in <float.h> etc, as those headers are
no longer at all relevant to its purpose. Unsurprisingly, a small number
of .c files turn out to have been depending on those inclusions, so add
them back in the .c files as needed.

So far, when a transaction with pending invalidations, but without an
assigned xid, committed, we simply ignored those invalidation
messages. That's problematic, because those are actually sent for a
reason.
Known symptoms of this include that existing sessions on a hot-standby
replica sometimes fail to notice new concurrently built indexes and
visibility map updates.
The solution is to WAL log such invalidations in transactions without an
xid. We considered to alternatively force-assign an xid, but that'd be
problematic for vacuum, which might be run in systems with few xids.
Important: This adds a new WAL record, but as the patch has to be
back-patched, we can't bump the WAL page magic. This means that standbys
have to be updated before primaries; otherwise
"PANIC: standby_redo: unknown op code 32" errors can be encountered.

Commit f3b5565dd4 was a couple of bricks shy
of a load; specifically, it missed putting pg_trigger_tgrelid_tgname_index
into the relcache init file, because that index is not used by any
syscache. However, we have historically nailed that index into cache for
performance reasons. The upshot was that load_relcache_init_file always
decided that the init file was busted and silently ignored it, resulting
in a significant hit to backend startup speed.
To fix, reinstantiate RelationIdIsInInitFile() as a wrapper around
RelationSupportsSysCache(), which can know about additional relations
that should be in the init file despite being unknown to syscache.c.
Also install some guards against future mistakes of this type: make
write_relcache_init_file Assert that all nailed relations get written to
the init file, and make load_relcache_init_file emit a WARNING if it takes
the "wrong number of nailed relations" exit path. Now that we remove the
init files during postmaster startup, that case should never occur in the
field, even if we are starting a minor-version update that added or removed
rels from the nailed set. So the warning shouldn't ever be seen by end
users, but it will show up in the regression tests if somebody breaks this
logic.
Back-patch to all supported branches, like the previous commit.
When we invalidate the relcache entry for a system catalog or index, we
must also delete the relcache "init file" if the init file contains a copy
of that rel's entry. The old way of doing this relied on a specially
maintained list of the OIDs of relations present in the init file: we made
the list either when reading the file in, or when writing the file out.
The problem is that when writing the file out, we included only rels
present in our local relcache, which might have already suffered some
deletions due to relcache inval events. In such cases we correctly decided
not to overwrite the real init file with incomplete data --- but we still
used the incomplete initFileRelationIds list for the rest of the current
session. This could result in wrong decisions about whether the session's
own actions require deletion of the init file, potentially allowing an init
file created by some other concurrent session to be left around even though
it's been made stale.
Since we don't support changing the schema of a system catalog at runtime,
the only likely scenario in which this would cause a problem in the field
involves a "vacuum full" on a catalog concurrently with other activity, and
even then it's far from easy to provoke. Remarkably, this has been broken
since 2002 (in commit 7863404417), but we had
never seen a reproducible test case until recently. If it did happen in
the field, the symptoms would probably involve unexpected "cache lookup
failed" errors to begin with, then "could not open file" failures after the
next checkpoint, as all accesses to the affected catalog stopped working.
Recovery would require manually removing the stale "pg_internal.init" file.
To fix, get rid of the initFileRelationIds list, and instead consult
syscache.c's list of relations used in catalog caches to decide whether a
relation is included in the init file. This should be a tad more efficient
anyway, since we're replacing linear search of a list with ~100 entries
with a binary search. It's a bit ugly that the init file contents are now
so directly tied to the catalog caches, but in practice that won't make
much difference.
Back-patch to all supported branches.
Instead of initializing a new TransInvalidationInfo for every
transaction or subtransaction, we can just do it for those
transactions or subtransactions that actually need to queue
invalidation messages. That also avoids needing to free those
entries at the end of a transaction or subtransaction that does
not generate any invalidation messages, which is by far the
common case.

Define padding bytes in SharedInvalidationMessage structs to be
defined. Otherwise the sinvaladt.c ringbuffer, which is accessed by
multiple processes, will cause spurious valgrind warnings about
undefined memory being used. That's because valgrind remembers the
undefined bytes from the last local process's store, not realizing
that another process has written since, filling the previously
uninitialized bytes.
This feature, building on previous commits, allows the write-ahead log
stream to be decoded into a series of logical changes; that is,
inserts, updates, and deletes and the transactions which contain them.
It is capable of handling decoding even across changes to the schema
of the effected tables. The output format is controlled by a
so-called "output plugin"; an example is included. To make use of
this in a real replication system, the output plugin will need to be
modified to produce output in the format appropriate to that system,
and to perform filtering.
Currently, information can be extracted from the logical decoding
system only via SQL; future commits will add the ability to stream
changes via walsender.

Although user-defined relations can't be directly created in
pg_catalog, it's possible for them to end up there, because you can
create them in some other schema and then use ALTER TABLE .. SET SCHEMA
to move them there. Previously, such relations couldn't afterwards
be manipulated, because IsSystemRelation()/IsSystemClass() rejected
all attempts to modify objects in the pg_catalog schema, regardless
of their origin. With this patch, they now reject only those
objects in pg_catalog which were created at initdb-time, allowing
most operations on user-created tables in pg_catalog to proceed
normally.
This patch also adds new functions IsCatalogRelation() and
IsCatalogClass(), which is similar to IsSystemRelation() and
IsSystemClass() but with a slightly narrower definition: only TOAST
tables of system catalogs are included, rather than *all* TOAST tables.
This is currently used only for making decisions about when
invalidation messages need to be sent, but upcoming logical decoding
patches will find other uses for this information.

Future patches are expected to introduce logical replication that
works by decoding WAL. WAL contains relfilenodes rather than relation
OIDs, so this infrastructure will be needed to find the relation OID
based on WAL contents.
If logical replication does not make it into this release, we probably
should consider reverting this, since it will add some overhead to DDL
operations that create new relations. One additional index insert per
pg_class row is not a large overhead, but it's more than zero.
Another way of meeting the needs of logical replication would be to
the relation OID to WAL, but that would burden DML operations, not
only DDL.

SnapshotNow scans have the undesirable property that, in the face of
concurrent updates, the scan can fail to see either the old or the new
versions of the row. In many cases, we work around this by requiring
DDL operations to hold AccessExclusiveLock on the object being
modified; in some cases, the existing locking is inadequate and random
failures occur as a result. This commit doesn't change anything
related to locking, but will hopefully pave the way to allowing lock
strength reductions in the future.
The major issue has held us back from making this change in the past
is that taking an MVCC snapshot is significantly more expensive than
using a static special snapshot such as SnapshotNow. However, testing
of various worst-case scenarios reveals that this problem is not
severe except under fairly extreme workloads. To mitigate those
problems, we avoid retaking the MVCC snapshot for each new scan;
instead, we take a new snapshot only when invalidation messages have
been processed. The catcache machinery already requires that
invalidation messages be sent before releasing the related heavyweight
lock; else other backends might rely on locally-cached data rather
than scanning the catalog at all. Thus, making snapshot reuse
dependent on the same guarantees shouldn't break anything that wasn't
already subtly broken.

This reduces unnecessary exposure of other headers through htup.h, which
is very widely included by many files.
I have chosen to move the function prototypes to the new file as well,
because that means htup.h no longer needs to include tupdesc.h. In
itself this doesn't have much effect in indirect inclusion of tupdesc.h
throughout the tree, because it's also required by execnodes.h; but it's
something to explore in the future, and it seemed best to do the htup.h
change now while I'm busy with it.
By my count there are 18 callers of CacheRegisterSyscacheCallback in the
core code in HEAD, so we are potentially leaving as few as 2 slots for any
add-on code to use (though possibly not all these callers would actually
activate in any particular session). That doesn't seem like a lot of
headroom, so let's pump it up a little.
This requires adjusting the API for syscache callback functions: they now
get a hash value, not a TID, to identify the target tuple. Most of them
weren't paying any attention to that argument anyway, but plancache did
require a small amount of fixing.
