Other code paths are already protected against this case, and _PG_init()
warns about that in pg_stat_statements.c. While on it, I have checked
the other extensions of the tree but did not notice any holes.

This commit adds "stats_reset" column into the pg_stat_statements_info
view. This column indicates the time at which all statistics in the
pg_stat_statements view were last reset.
Per discussion, this commit also changes pg_stat_statements_info code
so that "dealloc" column is reset at the same time as "stats_reset" is reset,
i.e., whenever all pg_stat_statements entries are removed, for the sake
of consistency. Previously "dealloc" was reset only when
pg_stat_statements_reset(0, 0, 0) is called and was not reset when
pg_stat_statements_reset() with non-zero value argument discards all
entries. This was confusing.

Invent a new flag bit HASH_STRINGS to specify C-string hashing, which
was formerly the default; and add assertions insisting that exactly
one of the bits HASH_STRINGS, HASH_BLOBS, and HASH_FUNCTION be set.
This is in hopes of preventing recurrences of the type of oversight
fixed in commit a1b8aa1e4 (i.e., mistakenly omitting HASH_BLOBS).
Also, when HASH_STRINGS is specified, insist that the keysize be
more than 8 bytes. This is a heuristic, but it should catch
accidental use of HASH_STRINGS for integer or pointer keys.
(Nearly all existing use-cases set the keysize to NAMEDATALEN or
more, so there's little reason to think this restriction should
be problematic.)
Tweak hash_create() to insist that the HASH_ELEM flag be set, and
remove the defaults it had for keysize and entrysize. Since those
defaults were undocumented and basically useless, no callers
omitted HASH_ELEM anyway.
Also, remove memset's zeroing the HASHCTL parameter struct from
those callers that had one. This has never been really necessary,
and while it wasn't a bad coding convention it was confusing that
some callers did it and some did not. We might as well save a few
cycles by standardizing on "not".
Also improve the documentation for hash_create().
In passing, improve reinit.c's usage of a hash table by storing
the key as a binary Oid rather than a string; and, since that's
a temporary hash table, allocate it in CurrentMemoryContext for
neatness.

If more distinct statements than pg_stat_statements.max are observed,
pg_stat_statements entries about the least-executed statements are
deallocated. This commit enables us to track the total number of times
those entries were deallocated. That number can be viewed in the
pg_stat_statements_info view that this commit adds. It's useful when
tuning pg_stat_statements.max parameter. If it's high, i.e., the entries
are deallocated very frequently, which might cause the performance
regression and we can increase pg_stat_statements.max to avoid those
frequent deallocations.
The pg_stat_statements_info view is intended to display the statistics
of pg_stat_statements module itself. Currently it has only one column
"dealloc" indicating the number of times entries were deallocated.
But an upcoming patch will add other columns (for example, the time
at which pg_stat_statements statistics were last reset) into the view.

Commit 6023b7ea71 allowed pg_stat_statements to track the number
of rows retrieved or affected by some utility commands including
CREATE MATERIALIZED VIEW. However it did not track the rowcount
of REFRESH MATERIALIZED VIEW. This commit allows pg_stat_statements
to track that.
To track that, this commit changes the query completion for
REFRESH MATERIALIZED VIEW so that it saves the rowcount. But note that
the rowcount is still not displayed in the command completion tag output.
That is, the display_rowcount flag of CMDTAG_REFRESH_MATERIALIZED_VIEW
command tag is left false in cmdtaglist.h. Otherwise, the change of
completion tag output might break applications using it.

Since v13 pg_stat_statements is allowed to track the planning time of
statements when track_planning option is enabled. Its default was on.
But this feature could cause more terrible spinlock contentions in
pg_stat_statements. As a result of this, Robins Tharakan reported that
v13 beta1 showed ~45% performance drop at high DB connection counts
(when compared with v12.3) during fully-cached SELECT-only test using
pgbench.
To avoid this performance regression by the default setting,
this commit changes default of pg_stat_statements.track_planning to off.
Back-patch to v13 where pg_stat_statements.track_planning was introduced.

Includes some manual cleanup of places that pgindent messed up,
most of which weren't per project style anyway.
Notably, it seems some people didn't absorb the style rules of
commit c9d297751, because there were a bunch of new occurrences
of function calls with a newline just after the left paren, all
with faulty expectations about how the rest of the call would get
indented.
In commit 33e05f89c5, we have added the option to display WAL usage
statistics in Explain and auto_explain. The display format used two spaces
between each field which is inconsistent with Buffer usage statistics which
is using one space between each field. Change the format to make WAL usage
statistics consistent with Buffer usage statistics.
This commit also changed the usage of "full page writes" to
"full page images" for WAL usage statistics to make it consistent with
other parts of code and docs.

This commit makes pg_stat_statements support new GUC
pg_stat_statements.track_planning. If this option is enabled,
pg_stat_statements tracks the planning statistics of the statements,
e.g., the number of times the statement was planned, the total time
spent planning the statement, etc. This feature is useful to check
the statements that it takes a long time to plan. Previously since
pg_stat_statements tracked only the execution statistics, we could
not use that for the purpose.
The planning and execution statistics are stored at the end of
each phase separately. So there are not always one-to-one relationship
between them. For example, if the statement is successfully planned
but fails in the execution phase, only its planning statistics are stored.
This may cause the users to be able to see different pg_stat_statements
results from the previous version. To avoid this,
pg_stat_statements.track_planning needs to be disabled.
This commit bumps the version of pg_stat_statements to 1.8
since it changes the definition of pg_stat_statements function.

Previously pg_stat_statements calculated the difference of buffer counters
by its own code even while BufferUsageAccumDiff() had the same code.
This commit expose BufferUsageAccumDiff() and makes pg_stat_statements
use it for the calculation, in order to simply the code.
This change also would be useful for the upcoming patch for the planning
counters in pg_stat_statements because the patch will add one more code
for the calculation of difference of buffer counters and that can easily be
done by using BufferUsageAccumDiff().

The backend was using strings to represent command tags and doing string
comparisons in multiple places, but that's slow and unhelpful. Create a
new command list with a supporting structure to use instead; this is
stored in a tag-list-file that can be tailored to specific purposes with
a caller-definable C macro, similar to what we do for WAL resource
managers. The first first such uses are a new CommandTag enum and a
CommandTagBehavior struct.
Replace numerous occurrences of char *completionTag with a
QueryCompletion struct so that the code no longer stores information
about completed queries in a cstring. Only at the last moment, in
EndCommand(), does this get converted to a string.
EventTriggerCacheItem no longer holds an array of palloc’d tag strings
in sorted order, but rather just a Bitmapset over the CommandTags.

access/hash is the place for the hash index AM, not really appropriate
for generic facilities, which is what hash_any is; having things the old
way meant that anything using hash_any had to include the AM's include
file, pointlessly polluting its namespace with unrelated, unnecessary
cruft.
Also move the HTEqual strategy number to access/stratnum.h from
access/hash.h.
To avoid breaking third-party extension code, add an #include
"utils/hashutils.h" to access/hash.h. (An easily removed line by
committers who enjoy their asbestos suits to protect them from angry
extension authors.)

This fixes two sets of issues related to the use of transient files in
the backend:
1) OpenTransientFile() has been used in some code paths with read-write
flags while read-only is sufficient, so switch those calls to be
read-only where necessary. These have been reported by Joe Conway.
2) When opening transient files, it is up to the caller to close the
file descriptors opened. In error code paths, CloseTransientFile() gets
called to clean up things before issuing an error. However in normal
exit paths, a lot of callers of CloseTransientFile() never actually
reported errors, which could leave a file descriptor open without
knowing about it. This is an issue I complained about a couple of
times, but never had the courage to write and submit a patch, so here we
go.
Note that one frontend code path is impacted by this commit so as an
error is issued when fetching control file data, making backend and
frontend to be treated consistently.

Historically we've always materialized the full output of a CTE query,
treating WITH as an optimization fence (so that, for example, restrictions
from the outer query cannot be pushed into it). This is appropriate when
the CTE query is INSERT/UPDATE/DELETE, or is recursive; but when the CTE
query is non-recursive and side-effect-free, there's no hazard of changing
the query results by pushing restrictions down.
Another argument for materialization is that it can avoid duplicate
computation of an expensive WITH query --- but that only applies if
the WITH query is called more than once in the outer query. Even then
it could still be a net loss, if each call has restrictions that
would allow just a small part of the WITH query to be computed.
Hence, let's change the behavior for WITH queries that are non-recursive
and side-effect-free. By default, we will inline them into the outer
query (removing the optimization fence) if they are called just once.
If they are called more than once, we will keep the old behavior by
default, but the user can override this and force inlining by specifying
NOT MATERIALIZED. Lastly, the user can force the old behavior by
specifying MATERIALIZED; this would mainly be useful when the query had
deliberately been employing WITH as an optimization fence to prevent a
poor choice of plan.

The fact that "SELECT expression" has no base relations has long been a
thorn in the side of the planner. It makes it hard to flatten a sub-query
that looks like that, or is a trivial VALUES() item, because the planner
generally uses relid sets to identify sub-relations, and such a sub-query
would have an empty relid set if we flattened it. prepjointree.c contains
some baroque logic that works around this in certain special cases --- but
there is a much better answer. We can replace an empty FROM clause with a
dummy RTE that acts like a table of one row and no columns, and then there
are no such corner cases to worry about. Instead we need some logic to
get rid of useless dummy RTEs, but that's simpler and covers more cases
than what was there before.
For really trivial cases, where the query is just "SELECT expression" and
nothing else, there's a hazard that adding the extra RTE makes for a
noticeable slowdown; even though it's not much processing, there's not
that much for the planner to do overall. However testing says that the
penalty is very small, close to the noise level. In more complex queries,
this is able to find optimizations that we could not find before.
The new RTE type is called RTE_RESULT, since the "scan" plan type it
gives rise to is a Result node (the same plan we produced for a "SELECT
expression" query before). To avoid confusion, rename the old ResultPath
path type to GroupResultPath, reflecting that it's only used in degenerate
grouping cases where we know the query produces just one grouped row.
(It wouldn't work to unify the two cases, because there are different
rules about where the associated quals live during query_planner.)
Note: although this touches readfuncs.c, I don't think a catversion
bump is required, because the added case can't occur in stored rules,
only plans.

The function pg_stat_statements_reset() is extended to accept userid, dbid,
and queryid as input parameters. Now, it can discard the statistics
gathered so far by pg_stat_statements corresponding to the specified
userid, dbid, and queryid. If no parameter is specified or all the
specified parameters have default value aka 0, it will discard all
statistics as per the old behavior.
The new behavior is useful to get the fresh statistics for a specific
user/database/query without resetting all the existing statistics.

Previously, ScanKeywordLookup was passed an array of string pointers.
This had some performance deficiencies: the strings themselves might
be scattered all over the place depending on the compiler (and some
quick checking shows that at least with gcc-on-Linux, they indeed
weren't reliably close together). That led to very cache-unfriendly
behavior as the binary search touched strings in many different pages.
Also, depending on the platform, the string pointers might need to
be adjusted at program start, so that they couldn't be simple constant
data. And the ScanKeyword struct had been designed with an eye to
32-bit machines originally; on 64-bit it requires 16 bytes per
keyword, making it even more cache-unfriendly.
Redesign so that the keyword strings themselves are allocated
consecutively (as part of one big char-string constant), thereby
eliminating the touch-lots-of-unrelated-pages syndrome. And get
rid of the ScanKeyword array in favor of three separate arrays:
uint16 offsets into the keyword array, uint16 token codes, and
uint8 keyword categories. That reduces the overhead per keyword
to 5 bytes instead of 16 (even less in programs that only need
one of the token codes and categories); moreover, the binary search
only touches the offsets array, further reducing its cache footprint.
This also lets us put the token codes somewhere else than the
keyword strings are, which avoids some unpleasant build dependencies.
While we're at it, wrap the data used by ScanKeywordLookup into
a struct that can be treated as an opaque type by most callers.
That doesn't change things much right now, but it will make it
less painful to switch to a hash-based lookup method, as is being
discussed in the mailing list thread.
Most of the change here is associated with adding a generator
script that can build the new data structure from the same
list-of-PG_KEYWORD header representation we used before.
The PG_KEYWORD lists that plpgsql and ecpg used to embed in
their scanner .c files have to be moved into headers, and the
Makefiles have to be taught to invoke the generator script.
This work is also necessary if we're to consider hash-based lookup,
since the generator script is what would be responsible for
constructing a hash table.
Aside from saving a few kilobytes in each program that includes
the keyword table, this seems to speed up raw parsing (flex+bison)
by a few percent. So it's worth doing even as it stands, though
we think we can gain even more with a follow-on patch to switch
to hash-based lookup.

Some error messages which report something about a file operation use
as well context which is already provided within the path being worked
on, making things rather duplicated. This creates more work for
translators, and does not actually bring clarity.
More could be done, however in a lot of cases the context used is
actually useful, still that patch gets down things with a good cut.

This takes advantage of the infrastructure introduced by commit
81c5e46c49 to greatly reduce the
likelihood that two different queries will end up with the same query
ID. It's still possible, of course, but whereas before it the chances
of a collision reached 25% around 50,000 queries, it will now take
more than 3 billion queries.
Backward incompatibility: Because the type exposed at the SQL level is
int8, users may now see negative query IDs in the pg_stat_statements
view (and also, query IDs more than 4 billion, which was the old
limit).

Allowing arrays with a domain type as their element type was left un-done
in the original domain patch, but not for any very good reason. This
omission leads to such surprising results as array_agg() not working on
a domain column, because the parser can't identify a suitable output type
for the polymorphic aggregate.
In order to fix this, first clean up the APIs of coerce_to_domain() and
some internal functions in parse_coerce.c so that we consistently pass
around a CoercionContext along with CoercionForm. Previously, we sometimes
passed an "isExplicit" boolean flag instead, which is strictly less
information; and coerce_to_domain() didn't even get that, but instead had
to reverse-engineer isExplicit from CoercionForm. That's contrary to the
documentation in primnodes.h that says that CoercionForm only affects
display and not semantics. I don't think this change fixes any live bugs,
but it makes things more consistent. The main reason for doing it though
is that now build_coercion_expression() receives ccontext, which it needs
in order to be able to recursively invoke coerce_to_target_type().
Next, reimplement ArrayCoerceExpr so that the node does not directly know
any details of what has to be done to the individual array elements while
performing the array coercion. Instead, the per-element processing is
represented by a sub-expression whose input is a source array element and
whose output is a target array element. This simplifies life in
parse_coerce.c, because it can build that sub-expression by a recursive
invocation of coerce_to_target_type(). The executor now handles the
per-element processing as a compiled expression instead of hard-wired code.
The main advantage of this is that we can use a single ArrayCoerceExpr to
handle as many as three successive steps per element: base type conversion,
typmod coercion, and domain constraint checking. The old code used two
stacked ArrayCoerceExprs to handle type + typmod coercion, which was pretty
inefficient, and adding yet another array deconstruction to do domain
constraint checking seemed very unappetizing.
In the case where we just need a single, very simple coercion function,
doing this straightforwardly leads to a noticeable increase in the
per-array-element runtime cost. Hence, add an additional shortcut evalfunc
in execExprInterp.c that skips unnecessary overhead for that specific form
of expression. The runtime speed of simple cases is within 1% or so of
where it was before, while cases that previously required two levels of
array processing are significantly faster.
Finally, create an implicit array type for every domain type, as we do for
base types, enums, etc. Everything except the array-coercion case seems
to just work without further effort.

The file handling functions from fd.c were called with a diverse mix of
notations for the file permissions when they were opening new files.
Almost all files created by the server should have the same permissions
set. So change the API so that e.g. OpenTransientFile() automatically
uses the standard permissions set, and OpenTransientFilePerm() is a new
function that takes an explicit permissions set for the few cases where
it is needed. This also saves an unnecessary argument for call sites
that are just opening an existing file.
While we're reviewing these APIs, get rid of the FileName typedef and
use the standard const char * for the file name and mode_t for the file
mode. This makes these functions match other file handling functions
and removes an unnecessary layer of mysteriousness. We can also get rid
of a few casts that way.

Add missing infrastructure for this node type, notably in ruleutils.c where
its lack could demonstrably cause EXPLAIN to fail. Add outfuncs/readfuncs
support. (outfuncs support is useful today for debugging purposes. The
readfuncs support may never be needed, since at present it would only
matter for parallel query and NextValueExpr should never appear in a
parallelizable query; but it seems like a bad idea to have a primnode type
that isn't fully supported here.) Teach planner infrastructure that
NextValueExpr is a volatile, parallel-unsafe, non-leaky expression node
with cost cpu_operator_cost. Given its limited scope of usage, there
*might* be no live bug today from the lack of that knowledge, but it's
certainly going to bite us on the rear someday. Teach pg_stat_statements
about the new node type, too.
While at it, also teach cost_qual_eval() that MinMaxExpr, SQLValueFunction,
XmlExpr, and CoerceToDomain should be charged as cpu_operator_cost.
Failing to do this for SQLValueFunction was an oversight in my commit
0bb51aa96. The others are longer-standing oversights, but no time like the
present to fix them. (In principle, CoerceToDomain could have cost much
higher than this, but it doesn't presently seem worth trying to examine the
domain's constraints here.)
Modify execExprInterp.c to execute NextValueExpr as an out-of-line
function; it seems quite unlikely to me that it's worth insisting that
it be inlined in all expression eval methods. Besides, providing the
out-of-line function doesn't stop anyone from inlining if they want to.
Adjust some places where NextValueExpr support had been inserted with the
aid of a dartboard rather than keeping it in the same order as elsewhere.

Don't move parenthesized lines to the left, even if that means they
flow past the right margin.
By default, BSD indent lines up statement continuation lines that are
within parentheses so that they start just to the right of the preceding
left parenthesis. However, traditionally, if that resulted in the
continuation line extending to the right of the desired right margin,
then indent would push it left just far enough to not overrun the margin,
if it could do so without making the continuation line start to the left of
the current statement indent. That makes for a weird mix of indentations
unless one has been completely rigid about never violating the 80-column
limit.
This behavior has been pretty universally panned by Postgres developers.
Hence, disable it with indent's new -lpl switch, so that parenthesized
lines are always lined up with the preceding left paren.
This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

Change pg_bsd_indent to follow upstream rules for placement of comments
to the right of code, and remove pgindent hack that caused comments
following #endif to not obey the general rule.
Commit e3860ffa4d wasn't actually using
the published version of pg_bsd_indent, but a hacked-up version that
tried to minimize the amount of movement of comments to the right of
code. The situation of interest is where such a comment has to be
moved to the right of its default placement at column 33 because there's
code there. BSD indent has always moved right in units of tab stops
in such cases --- but in the previous incarnation, indent was working
in 8-space tab stops, while now it knows we use 4-space tabs. So the
net result is that in about half the cases, such comments are placed
one tab stop left of before. This is better all around: it leaves
more room on the line for comment text, and it means that in such
cases the comment uniformly starts at the next 4-space tab stop after
the code, rather than sometimes one and sometimes two tabs after.
Also, ensure that comments following #endif are indented the same
as comments following other preprocessor commands such as #else.
That inconsistency turns out to have been self-inflicted damage
from a poorly-thought-through post-indent "fixup" in pgindent.
