This adds a new optional support function to the GiST access method:
sortsupport. If it is defined, the GiST index is built by sorting all data
to the order defined by the sortsupport's comparator function, and packing
the tuples in that order to GiST pages. This is similar to how B-tree
index build works, and is much faster than inserting the tuples one by
one. The resulting index is smaller too, because the pages are packed more
tightly, upto 'fillfactor'. The normal build method works by splitting
pages, which tends to lead to more wasted space.
The quality of the resulting index depends on how good the opclass-defined
sort order is. A good order preserves locality of the input data.
As the first user of this facility, add 'sortsupport' function to the
point_ops opclass. It sorts the points in Z-order (aka Morton Code), by
interleaving the bits of the X and Y coordinates.

The following renaming is done so as source files related to index
access methods are more consistent with table access methods (the
original names used for index AMs ware too generic, and could be
confused as including features related to table AMs):
- amapi.h -> indexam.h.
- amapi.c -> indexamapi.c. Here we have an equivalent with
backend/access/table/tableamapi.c.
- amvalidate.c -> indexamvalidate.c.
- amvalidate.h -> indexamvalidate.h.
- genam.c -> indexgenam.c.
- genam.h -> indexgenam.h.
This has been discussed during the development of v12 when table AM was
worked on, but the renaming never happened.

All our current in core relation options of type string (not many,
admittedly) behave in reality like enums. But after seeing an
implementation for enum reloptions, it's clear that strings are messier,
so introduce the new reloption type. Switch all string options to be
enums instead.
Fortunately we have a recently introduced test module for reloptions, so
we don't lose coverage of string reloptions, which may still be used by
third-party modules.

Instead of WAL-logging every modification during the build separately,
first build the index without any WAL-logging, and make a separate pass
through the index at the end, to write all pages to the WAL. This
significantly reduces the amount of WAL generated, and is usually also
faster, despite the extra I/O needed for the extra scan through the index.
WAL generated this way is also faster to replay.
For GiST, the LSN-NSN interlock makes this a little tricky. All pages must
be marked with a valid (i.e. non-zero) LSN, so that the parent-child
LSN-NSN interlock works correctly. We now use magic value 1 for that during
index build. Change the fake LSN counter to begin from 1000, so that 1 is
safely smaller than any real or fake LSN. 2 would've been enough for our
purposes, but let's reserve a bigger range, in case we need more special
values in the future.

This uses the progress reporting infrastructure added by c16dc1aca5,
adding support for CREATE INDEX and CREATE INDEX CONCURRENTLY.
There are two pieces to this: one is index-AM-agnostic, and the other is
AM-specific. The latter is fairly elaborate for btrees, including
reportage for parallel index builds and the separate phases that btree
index creation uses; other index AMs, which are much simpler in their
building procedures, have simplistic reporting only, but that seems
sufficient, at least for non-concurrent builds.
The index-AM-agnostic part is fairly complete, providing insight into
the CONCURRENTLY wait phases as well as block-based progress during the
index validation table scan. (The index validation index scan requires
patching each AM, which has not been included here.)

To support building indexes over tables of different AMs, the scans to
do so need to be routed through the table AM. While moving a fair
amount of code, nearly all the changes are just moving code to below a
callback.
Currently the range based interface wouldn't make much sense for non
block based table AMs. But that seems aceptable for now.

Create a new header optimizer/optimizer.h, which exposes just the
planner functions that can be used "at arm's length", without need
to access Paths or the other planner-internal data structures defined
in nodes/relation.h. This is intended to provide the whole planner
API seen by most of the rest of the system; although FDWs still need
to use additional stuff, and more thought is also needed about just
what selfuncs.c should rely on.
The main point of doing this now is to limit the amount of new
#include baggage that will be needed by "planner support functions",
which I expect to introduce later, and which will be in relevant
datatype modules rather than anywhere near the planner.
This commit just moves relevant declarations into optimizer.h from
other header files (a couple of which go away because everything
got moved), and adjusts #include lists to match. There's further
cleanup that could be done if we want to decide that some stuff
being exposed by optimizer.h doesn't belong in the planner at all,
but I'll leave that for another day.

013ebc0a7b implements so-called GiST microvacuum. That is gistgettuple() marks
index tuples as dead when kill_prior_tuple is set. Later, when new tuple
insertion claims page space, those dead index tuples are physically deleted
from page. When this deletion is replayed on standby, it might conflict with
read-only queries. But 013ebc0a7b doesn't handle this. That may lead to
disappearance of some tuples from read-only snapshots on standby.
This commit implements resolving of conflicts between replay of GiST microvacuum
and standby queries. On the master we implement new WAL record type
XLOG_GIST_DELETE, which comprises necessary information. On stable releases
we've to be tricky to keep WAL compatibility. Information required for conflict
processing is just appended to data of XLOG_GIST_PAGE_UPDATE record. So,
PostgreSQL version, which doesn't know about conflict processing, will just
ignore that.

To make this work, tuplesort.c and logtape.c must also support
parallelism, so this patch adds that infrastructure and then applies
it to the particular case of parallel btree index builds. Testing
to date shows that this can often be 2-3x faster than a serial
index build.
The model for deciding how many workers to use is fairly primitive
at present, but it's better than not having the feature. We can
refine it as we get more experience.

This is a mechanical change in preparation for a later commit that
will change the layout of TupleDesc. Introducing a macro to abstract
the details of where attributes are stored will allow us to change
that in separate step and revise it in future.

Don't move parenthesized lines to the left, even if that means they
flow past the right margin.
By default, BSD indent lines up statement continuation lines that are
within parentheses so that they start just to the right of the preceding
left parenthesis. However, traditionally, if that resulted in the
continuation line extending to the right of the desired right margin,
then indent would push it left just far enough to not overrun the margin,
if it could do so without making the continuation line start to the left of
the current statement indent. That makes for a weird mix of indentations
unless one has been completely rigid about never violating the 80-column
limit.
This behavior has been pretty universally panned by Postgres developers.
Hence, disable it with indent's new -lpl switch, so that parenthesized
lines are always lined up with the preceding left paren.
This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

Change pg_bsd_indent to follow upstream rules for placement of comments
to the right of code, and remove pgindent hack that caused comments
following #endif to not obey the general rule.
Commit e3860ffa4d wasn't actually using
the published version of pg_bsd_indent, but a hacked-up version that
tried to minimize the amount of movement of comments to the right of
code. The situation of interest is where such a comment has to be
moved to the right of its default placement at column 33 because there's
code there. BSD indent has always moved right in units of tab stops
in such cases --- but in the previous incarnation, indent was working
in 8-space tab stops, while now it knows we use 4-space tabs. So the
net result is that in about half the cases, such comments are placed
one tab stop left of before. This is better all around: it leaves
more room on the line for comment text, and it means that in such
cases the comment uniformly starts at the next 4-space tab stop after
the code, rather than sometimes one and sometimes two tabs after.
Also, ensure that comments following #endif are indented the same
as comments following other preprocessor commands such as #else.
That inconsistency turns out to have been self-inflicted damage
from a poorly-thought-through post-indent "fixup" in pgindent.
This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

The xlog-specific headers need to be included in both frontend code -
specifically, pg_waldump - and the backend, but the remainder of the
private headers for each index are only needed by the backend. By
splitting the xlog stuff out into separate headers, pg_waldump pulls
in fewer backend headers, which is a good thing.

The reverted changes were intended to force a choice of whether any
newly-added BufferGetPage() calls needed to be accompanied by a
test of the snapshot age, to support the "snapshot too old"
feature. Such an accompanying test is needed in about 7% of the
cases, where the page is being used as part of a scan rather than
positioning for other purposes (such as DML or vacuuming). The
additional effort required for back-patching, and the doubt whether
the intended benefit would really be there, have indicated it is
best just to rely on developers to do the right thing based on
comments and existing usage, as we do with many other conventions.
This change should have little or no effect on generated executable
code.
Motivated by the back-patching pain of Tom Lane and Robert Haas
This patch is a no-op patch which is intended to reduce the chances
of failures of omission once the functional part of the "snapshot
too old" patch goes in. It adds parameters for snapshot, relation,
and an enum to specify whether the snapshot age check needs to be
done for the page at this point. This initial patch passes NULL
for the first two new parameters and BGP_NO_SNAPSHOT_TEST for the
third. The follow-on patch will change the places where the test
needs to be made.
This patch reduces pg_am to just two columns, a name and a handler
function. All the data formerly obtained from pg_am is now provided
in a C struct returned by the handler function. This is similar to
the designs we've adopted for FDWs and tablesample methods. There
are multiple advantages. For one, the index AM's support functions
are now simple C functions, making them faster to call and much less
error-prone, since the C compiler can now check function signatures.
For another, this will make it far more practical to define index access
methods in installable extensions.
A disadvantage is that SQL-level code can no longer see attributes
of index AMs; in particular, some of the crosschecks in the opr_sanity
regression test are no longer possible from SQL. We've addressed that
by adding a facility for the index AM to perform such checks instead.
(Much more could be done in that line, but for now we're content if the
amvalidate functions more or less replace what opr_sanity used to do.)
We might also want to expose some sort of reporting functionality, but
this patch doesn't do that.

Previously, if you wanted anything besides C-string hash keys, you had to
specify a custom hashing function to hash_create(). Nearly all such
callers were specifying tag_hash or oid_hash; which is tedious, and rather
error-prone, since a caller could easily miss the opportunity to optimize
by using hash_uint32 when appropriate. Replace this with a design whereby
callers using simple binary-data keys just specify HASH_BLOBS and don't
need to mess with specific support functions. hash_create() itself will
take care of optimizing when the key size is four bytes.
This nets out saving a few hundred bytes of code space, and offers
a measurable performance improvement in tidbitmap.c (which was not
exploiting the opportunity to use hash_uint32 for its 4-byte keys).
There might be some wins elsewhere too, I didn't analyze closely.
In future we could look into offering a similar optimized hashing function
for 8-byte keys. Under this design that could be done in a centralized
and machine-independent fashion, whereas getting it right for keys of
platform-dependent sizes would've been notationally painful before.
For the moment, the old way still works fine, so as not to break source
code compatibility for loadable modules. Eventually we might want to
remove tag_hash and friends from the exported API altogether, since there's
no real need for them to be explicitly referenced from outside dynahash.c.

Each WAL record now carries information about the modified relation and
block(s) in a standardized format. That makes it easier to write tools that
need that information, like pg_rewind, prefetching the blocks to speed up
recovery, etc.
There's a whole new API for building WAL records, replacing the XLogRecData
chains used previously. The new API consists of XLogRegister* functions,
which are called for each buffer and chunk of data that is added to the
record. The new API also gives more control over when a full-page image is
written, by passing flags to the XLogRegisterBuffer function.
This also simplifies the XLogReadBufferForRedo() calls. The function can dig
the relation and block number from the WAL record, so they no longer need to
be passed as arguments.
For the convenience of redo routines, XLogReader now disects each WAL record
after reading it, copying the main data part and the per-block data into
MAXALIGNed buffers. The data chunks are not aligned within the WAL record,
but the redo routines can assume that the pointers returned by XLogRecGet*
functions are. Redo routines are now passed the XLogReaderState, which
contains the record in the already-disected format, instead of the plain
XLogRecord.
The new record format also makes the fixed size XLogRecord header smaller,
by removing the xl_len field. The length of the "main data" portion is now
stored at the end of the WAL record, and there's a separate header after
XLogRecord for it. The alignment padding at the end of XLogRecord is also
removed. This compansates for the fact that the new format would otherwise
be more bulky than the old format.

xlog.c is huge, this makes it a little bit smaller, which is nice. Functions
related to putting together the WAL record are in xloginsert.c, and the
lower level stuff for managing WAL buffers and such are in xlog.c.
Also move the definition of XLogRecord to a separate header file. This
causes churn in the #includes of all the files that write WAL records, and
redo routines, but it avoids pulling in xlog.h into most places.

Make slightly better decisions about indentation than what pgindent
is capable of. Mostly breaking out long function calls into one
line per argument, with a few other minor adjustments.
No functional changes- all whitespace.
pgindent ran cleanly (didn't change anything) after.
Passes all regressions.
Remove use of PageSetTLI() from all page manipulation functions
and adjust README to indicate change in the way we make changes
to pages. Repurpose those bytes into the pd_checksum field and
explain how that works in comments about page header.

The reason this wasn't supported before was that GiST indexes need an
increasing sequence to detect concurrent page-splits. In a regular WAL-
logged GiST index, the LSN of the page-split record is used for that
purpose, and in a temporary index, we can get away with a backend-local
counter. Neither of those methods works for an unlogged relation.
To provide such an increasing sequence of numbers, create a "fake LSN"
counter that is saved and restored across shutdowns. On recovery, unlogged
relations are blown away, so the counter doesn't need to survive that
either.

We use a hash table to track the parents of inner pages, but when inserting
to a leaf page, the caller of gistbufferinginserttuples() must pass a
correct block number of the leaf's parent page. Before gistProcessItup()
descends to a child page, it checks if the downlink needs to be adjusted to
accommodate the new tuple, and updates the downlink if necessary. However,
updating the downlink might require splitting the page, which might move the
downlink to a page to the right. gistProcessItup() doesn't realize that, so
when it descends to the leaf page, it might pass an out-of-date parent block
number as a result. Fix that by returning the block a tuple was inserted to
from gistbufferinginserttuples().
This fixes the bug reported by Zdeněk Jílovec.
We used to mimic the way a stack is constructed when descending the tree
during normal GiST inserts, but that was quite complicated during a buffered
build. It was also wrong: in GiST, the left-to-right relationships on
different levels might not match each other, so that when you know the
parent of a child page, you won't necessarily find the parent of the page to
the right of the child page by following the rightlinks at the parent level.
This sometimes led to "could not re-find parent" errors while building a
GiST index.
We now use a simple hash table to track the parent of every internal page.
Whenever a page is split, and downlinks are moved from one page to another,
we update the hash table accordingly. This is also better for performance
than the old method, as we never need to move right to re-find the parent
page, which could take a significant amount of time for buffers that were
created much earlier in the index build.
There were two bugs here: We forgot to call gistFreeBuildBuffers() function
at the end of build, and we passed interXact == true to BufFileCreateTemp,
so the file wasn't automatically cleaned up at end-of-transaction either.
The result of (maintenance_work_mem * 1024) / BLCKSZ doesn't fit in a signed
32-bit integer, if maintenance_work_mem >= 2GB. Use double instead. And
while we're at it, write the calculations in an easier to understand form,
with the intermediary steps written out and commented.
pg_trgm was already doing this unofficially, but the implementation hadn't
been thought through very well and leaked memory. Restructure the core
GiST code so that it actually works, and document it. Ordinarily this
would have required an extra memory context creation/destruction for each
GiST index search, but I was able to avoid that in the normal case of a
non-rescanned search by finessing the handling of the RBTree. It used to
have its own context always, but now shares a context with the
scan-lifespan data structures, unless there is more than one rescan call.
This should make the added overhead unnoticeable in typical cases.
queuedForEmptying flag correctly on buffer when adding it to the queue.
Also, don't add buffer to the queue if it's there already. These were
harmless oversights; failing to set the flag just means that a buffer might
get added to the queue twice if more tuples are added to it (although that
can't actually happen at this point because all the upper buffers have
already been emptied), and having the same buffer twice in the emptying
queue is harmless. But better be tidy.
When building a GiST index that doesn't fit in cache, buffers are attached
to some internal nodes in the index. This speeds up the build by avoiding
random I/O that would otherwise be needed to traverse all the way down the
tree to the find right leaf page for tuple.
