Quotes are applied to GUCs in a very inconsistent way across the code
base, with a mix of double quotes or no quotes used. This commit
removes double quotes around all the GUC names that are obviously
referred to as parameters with non-English words (use of underscore,
mixed case, etc).
This is the result of a discussion with Álvaro Herrera, Nathan Bossart,
Laurenz Albe, Peter Eisentraut, Tom Lane and Daniel Gustafsson.

We've had repeated bugs in the area of handling SLRU wraparound in the past,
some of which have caused data loss. Switching to an indexing system for SLRUs
that does not wrap around should allow us to get rid of a whole bunch
of problems and improve the overall reliability of the system.
This particular patch however only changes the indexing and doesn't address
the wraparound per se. This is going to be done in the following patches.

This shaves some code by replacing the combinations of
CreateTemplateTupleDesc()/TupleDescInitEntry() hardcoding a mapping of
the attributes listed in pg_proc.dat by get_call_result_type() to build
the TupleDesc needed for the rows generated.
get_call_result_type() is more expensive than the former style, but this
removes some duplication with the lists of OUT parameters (pg_proc.dat
and the attributes hardcoded in these code paths). This is applied to
functions that are not considered as critical (aka that could be called
repeatedly for monitoring purposes).

xlog.h is directly and indirectly #included in a lot of places. With
this change, xloginsert.h is no longer unnecessarily included in the
large number of them that don't need it.

The logic in charge of writing commit timestamps (enabled with
track_commit_timestamp) for subtransactions had a one-bug bug,
where it would be possible that commit timestamps go missing for the
last subtransaction committed.
While on it, simplify a bit the iteration logic in the loop writing the
commit timestamps, as per suggestions from Kyotaro Horiguchi and Tom
Lane, so as some variable initializations are not part of the loop
itself.

Commit 438fc4a39c prevented the WAL replay from writing
COMMIT_TS_SETTS record. By this change there is no code that
generates COMMIT_TS_SETTS record in PostgreSQL core.
Also we can think that there are no extensions using the record
because we've not received so far any complaints about the issue
that commit 438fc4a39c fixed. Therefore this commit removes
COMMIT_TS_SETTS record and its related code. Even without
this record, the timestamp required for commit timestamp feature
can be acquired from the COMMIT record.
Bump WAL page magic.

Previously the WAL replay of COMMIT_TS_SETTS record called
TransactionTreeSetCommitTsData() with the argument write_xlog=true,
which generated and wrote new COMMIT_TS_SETTS record.
This should not be acceptable because it's during recovery.
This commit fixes the WAL replay of COMMIT_TS_SETTS record
so that it calls TransactionTreeSetCommitTsData() with write_xlog=false
and doesn't generate new WAL during recovery.
Back-patch to all supported branches.

Every core SLRU wraps around. With the exception of pg_notify, the wrap
point can fall in the middle of a page. Account for this in the
PagePrecedes callback specification and in SimpleLruTruncate()'s use of
said callback. Update each callback implementation to fit the new
specification. This changes SerialPagePrecedesLogically() from the
style of asyncQueuePagePrecedes() to the style of CLOGPagePrecedes().
(Whereas pg_clog and pg_serial share a key space, pg_serial is nothing
like pg_notify.) The bug fixed here has the same symptoms and user
followup steps as 592a589a04. Back-patch
to 9.5 (all supported versions).

Some places were using PG_GETARG_UINT32 where PG_GETARG_TRANSACTIONID
would be more appropriate. (Of course, they are the same internally,
so there is no externally visible effect.) To do that, export
PG_GETARG_TRANSACTIONID outside of xid.c. We also export
PG_RETURN_TRANSACTIONID for symmetry, even though there are currently
no external users.

Previously, we called fsync() after writing out individual pg_xact,
pg_multixact and pg_commit_ts pages due to cache pressure, leading to
regular I/O stalls in user backends and recovery. Collapse requests for
the same file into a single system call as part of the next checkpoint,
as we already did for relation files, using the infrastructure developed
by commit 3eb77eba. This can cause a significant improvement to
recovery performance, especially when it's otherwise CPU-bound.
Hoist ProcessSyncRequests() up into CheckPointGuts() to make it clearer
that it applies to all the SLRU mini-buffer-pools as well as the main
buffer pool. Rearrange things so that data collected in CheckpointStats
includes SLRU activity.
Also remove the Shutdown{CLOG,CommitTS,SUBTRANS,MultiXact}() functions,
because they were redundant after the shutdown checkpoint that
immediately precedes them. (I'm not sure if they were ever needed, but
they aren't now.)

Harmonize behavior by moving reponsibility for fsyncing directories down
into slru.c. In 10 and later, only the multixact directories were
missed (see commit 1b02be21), and in older branches all SLRUs were
missed.

Including Full in variable names duplicates the type information and
leads to overly long names. As FullTransactionId cannot accidentally
be casted to TransactionId that does not seem necessary.

This includes two changes:
- Addition of a new function pg_xact_commit_timestamp_origin() able, for
a given transaction ID, to return the commit timestamp and replication
origin of this transaction. An equivalent function existed in
pglogical.
- Addition of the replication origin to pg_last_committed_xact().
The commit timestamp manager includes already APIs able to return the
replication origin of a transaction on top of its commit timestamp, but
the code paths for replication origins were never stressed as those
functions have never looked for a replication origin, and the SQL
functions available have never included this information since their
introduction in 73c986a.
While on it, refactor a test of modules/commit_ts/ to use tstzrange() to
check that a transaction timestamp is within the wanted range, making
the test a bit easier to read.
Bump catalog version.

Originally, the names assigned to SLRUs had no purpose other than
being shmem lookup keys, so not a lot of thought went into them.
As of v13, though, we're exposing them in the pg_stat_slru view and
the pg_stat_reset_slru function, so it seems advisable to take a bit
more care. Rename them to names based on the associated on-disk
storage directories (which fortunately we *did* think about, to some
extent; since those are also visible to DBAs, consistency seems like
a good thing). Also rename the associated LWLocks, since those names
are likewise user-exposed now as wait event names.
For the most part I only touched symbols used in the respective modules'
SimpleLruInit() calls, not the names of other related objects. This
renaming could have been taken further, and maybe someday we will do so.
But for now it seems undesirable to change the names of any globally
visible functions or structs, so some inconsistency is unavoidable.
(But I *did* terminate "oldserxid" with prejudice, as I found that
name both unreadable and not descriptive of the SLRU's contents.)
Table 27.12 needs re-alphabetization now, but I'll leave that till
after the other LWLock renamings I have in mind.

Instead of inferring epoch progress from xids and checkpoints,
introduce a 64 bit FullTransactionId type and use it to track xid
generation. This fixes an unlikely bug where the epoch is reported
incorrectly if the range of active xids wraps around more than once
between checkpoints.
The only user-visible effect of this commit is to correct the epoch
used by txid_current() and txid_status(), also visible with
pg_controldata, in those rare circumstances. It also creates some
basic infrastructure so that later patches can use 64 bit
transaction IDs in more places.
The new type is a struct that we pass by value, as a form of strong
typedef. This prevents the sort of accidental confusion between
TransactionId and FullTransactionId that would be possible if we
were to use a plain old uint64.

Previously tables declared WITH OIDS, including a significant fraction
of the catalog tables, stored the oid column not as a normal column,
but as part of the tuple header.
This special column was not shown by default, which was somewhat odd,
as it's often (consider e.g. pg_class.oid) one of the more important
parts of a row. Neither pg_dump nor COPY included the contents of the
oid column by default.
The fact that the oid column was not an ordinary column necessitated a
significant amount of special case code to support oid columns. That
already was painful for the existing, but upcoming work aiming to make
table storage pluggable, would have required expanding and duplicating
that "specialness" significantly.
WITH OIDS has been deprecated since 2005 (commit ff02d0a05280e0).
Remove it.
Removing includes:
- CREATE TABLE and ALTER TABLE syntax for declaring the table to be
WITH OIDS has been removed (WITH (oids[ = true]) will error out)
- pg_dump does not support dumping tables declared WITH OIDS and will
issue a warning when dumping one (and ignore the oid column).
- restoring an pg_dump archive with pg_restore will warn when
restoring a table with oid contents (and ignore the oid column)
- COPY will refuse to load binary dump that includes oids.
- pg_upgrade will error out when encountering tables declared WITH
OIDS, they have to be altered to remove the oid column first.
- Functionality to access the oid of the last inserted row (like
plpgsql's RESULT_OID, spi's SPI_lastoid, ...) has been removed.
The syntax for declaring a table WITHOUT OIDS (or WITH (oids = false)
for CREATE TABLE) is still supported. While that requires a bit of
support code, it seems unnecessary to break applications / dumps that
do not use oids, and are explicit about not using them.
The biggest user of WITH OID columns was postgres' catalog. This
commit changes all 'magic' oid columns to be columns that are normally
declared and stored. To reduce unnecessary query breakage all the
newly added columns are still named 'oid', even if a table's column
naming scheme would indicate 'reloid' or such. This obviously
requires adapting a lot code, mostly replacing oid access via
HeapTupleGetOid() with access to the underlying Form_pg_*->oid column.
The bootstrap process now assigns oids for all oid columns in
genbki.pl that do not have an explicit value (starting at the largest
oid previously used), only oids assigned later by oids will be above
FirstBootstrapObjectId. As the oid column now is a normal column the
special bootstrap syntax for oids has been removed.
Oids are not automatically assigned during insertion anymore, all
backend code explicitly assigns oids with GetNewOidWithIndex(). For
the rare case that insertions into the catalog via SQL are called for
the new pg_nextoid() function can be used (which only works on catalog
tables).
The fact that oid columns on system tables are now normal columns
means that they will be included in the set of columns expanded
by * (i.e. SELECT * FROM pg_class will now include the table's oid,
previously it did not). It'd not technically be hard to hide oid
column by default, but that'd mean confusing behavior would either
have to be carried forward forever, or it'd cause breakage down the
line.
While it's not unlikely that further adjustments are needed, the
scope/invasiveness of the patch makes it worthwhile to get merge this
now. It's painful to maintain externally, too complicated to commit
after the code code freeze, and a dependency of a number of other
patches.

The activation and deactivation of commit timestamp tracking has not
been handled consistently for a primary or standbys at recovery. The
facility can be activated at three different moments of recovery:
- The beginning, where a primary would use the GUC value for the
decision-making, and where a standby relies on the contents of the
control file.
- When replaying a XLOG_PARAMETER_CHANGE record at redo.
- The end, where both primary and standby rely on the GUC value.
Using the GUC value for a primary at the beginning of recovery causes
problems with commit timestamp access when doing crash recovery.
Particularly, when replaying transaction commits, it could be possible
that an attempt to read commit timestamps is done for a transaction
which committed at a moment when track_commit_timestamp was disabled.
A test case is added to reproduce the failure. The test works down to
v11 as it takes advantage of transaction commits within procedures.

The lower case spellings are C and C++ standard and are used in most
parts of the PostgreSQL sources. The upper case spellings are only used
in some files/modules. So standardize on the standard spellings.
The APIs for ICU, Perl, and Windows define their own TRUE and FALSE, so
those are left as is when using those APIs.
In code comments, we use the lower-case spelling for the C concepts and
keep the upper-case spelling for the SQL concepts.

Don't move parenthesized lines to the left, even if that means they
flow past the right margin.
By default, BSD indent lines up statement continuation lines that are
within parentheses so that they start just to the right of the preceding
left parenthesis. However, traditionally, if that resulted in the
continuation line extending to the right of the desired right margin,
then indent would push it left just far enough to not overrun the margin,
if it could do so without making the continuation line start to the left of
the current statement indent. That makes for a weird mix of indentations
unless one has been completely rigid about never violating the 80-column
limit.
This behavior has been pretty universally panned by Postgres developers.
Hence, disable it with indent's new -lpl switch, so that parenthesized
lines are always lined up with the preceding left paren.
This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

If file was created/deleted just before powerloss it's possible that
file system will miss that. To prevent it, call fsync() where creating/
unlinkg file is critical.

If a user requests the commit timestamp for a transaction old enough
that its data is concurrently being truncated away by vacuum at just the
right time, they would receive an ugly internal file-not-found error
message from slru.c rather than the expected NULL return value.
In a primary server, the window for the race is very small: the lookup
has to occur exactly between the two calls by vacuum, and there's not a
lot that happens between them (mostly just a multixact truncate). In a
standby server, however, the window is larger because the truncation is
executed as soon as the WAL record for it is replayed, but the advance
of the oldest-Xid is not executed until the next checkpoint record.
To fix in the primary, simply reverse the order of operations in
vac_truncate_clog. To fix in the standby, augment the WAL truncation
record so that the standby is aware of the new oldest-XID value and can
apply the update immediately. WAL version bumped because of this.
No backpatch, because of the low importance of the bug and its rarity.

Previously, requesting commit timestamp for transactions
FrozenTransactionId and BootstrapTransactionId resulted in an error.
But since those values can validly appear in committed tuples' Xmin,
this behavior is unhelpful and error prone: each caller would have to
special-case those values before requesting timestamp data for an Xid.
We already have a perfectly good interface for returning "the Xid you
requested is too old for us to have commit TS data for it", so let's use
that instead.
Backpatch to 9.5, where commit timestamps appeared.

lockdefs.h was only split from lock.h relatively recently, and
represents a minimal subset of the old lock.h. heapam.h only needs
that smaller subset, so adjust it to include only that. This requires
some corresponding adjustments elsewhere.

The variables newestCommitTs and oldestCommitTs sound as if they are
timestamps, but in fact they are the transaction Ids that correspond
to the newest and oldest timestamps rather than the actual timestamps.
Rename these variables to reflect that they are actually xids: to wit
newestCommitTsXid and oldestCommitTsXid respectively. Also modify
related code in a similar fashion, particularly the user facing output
emitted by pg_controldata and pg_resetxlog.

This module needs explicit initialization in order to replay WAL records
in recovery, but we had broken this recently following changes to make
other (stranger) scenarios work correctly. To fix, rework the
initialization sequence so that it always takes place before WAL replay
commences for both master and standby.
I could have gone for a more localized fix that just added a "startup"
call for the master server, but it seemed better to restructure the
existing callers as well so that the whole thing made more sense. As a
drawback, there is more control logic in xlog.c now than previously, but
doing otherwise meant passing down the ControlFile flag, which seemed
uglier as a whole.
This also meant adding a check to not re-execute ActivateCommitTs if it
had already been called.
