When evaluating clauses on multiple scan keys of a multi-column BRIN
index, we can stop processing as soon as we find a scan key eliminating
the range, and the range should not be added to tbe bitmap.
That's how it worked before 14, but since a681e3c107 the code treated
the range as matching if it matched at least the last scan key.
Backpatch to 14, where this code was introduced.

Instead of dozens of mostly-duplicate pg_foo_ownercheck() functions,
write one common function object_ownercheck() that can handle almost
all of them. We already have all the information we need, such as
which system catalog corresponds to which catalog table and which
column is the owner column.

guc.c has grown to be one of our largest .c files, making it
a bottleneck for compilation. It's also acquired a bunch of
knowledge that'd be better kept elsewhere, because of our not
very good habit of putting variable-specific check hooks here.
Hence, split it up along these lines:
* guc.c itself retains just the core GUC housekeeping mechanisms.
* New file guc_funcs.c contains the SET/SHOW interfaces and some
SQL-accessible functions for GUC manipulation.
* New file guc_tables.c contains the data arrays that define the
built-in GUC variables, along with some already-exported constant
tables.
* GUC check/assign/show hook functions are moved to the variable's
home module, whenever that's clearly identifiable. A few hard-
to-classify hooks ended up in commands/variable.c, which was
already a home for miscellaneous GUC hook functions.
To avoid cluttering a lot more header files with #include "guc.h",
I also invented a new header file utils/guc_hooks.h and put all
the GUC hook functions' declarations there, regardless of their
originating module. That allowed removal of #include "guc.h"
from some existing headers. The fallout from that (hopefully
all caught here) demonstrates clearly why such inclusions are
best minimized: there are a lot of files that, for example,
were getting array.h at two or more levels of remove, despite
not having any connection at all to GUCs in themselves.
There is some very minor code beautification here, such as
renaming a couple of inconsistently-named hook functions
and improving some comments. But mostly this just moves
code from point A to point B and deals with the ensuing
needs for #include adjustments and exporting a few functions
that previously weren't exported.

In a similar effort to f01592f91, here we're targetting fixing the
warnings that -Wshadow=compatible-local produces that we can fix by moving
a variable to an inner scope to stop that variable from being shadowed by
another variable declared somewhere later in the function.
All of the warnings being fixed here are changing the scope of variables
which are being used as an iterator for a "for" loop. In each instance,
the fix happens to be changing the for loop to use the C99 type
initialization. Much of this code likely pre-dates our use of C99.
Reducing the scope of the outer scoped variable seems like the safest way
to fix these. Renaming seems more likely to risk patches using the wrong
variable. Reducing the scope is more likely to result in a compilation
failure after applying some future patch rather than introducing bugs with
it.
By my count, this takes the warning count from 129 down to 114.

This reverts commits 5753d4ee32 and fe60b67250 that modified HOT to
ignore BRIN indexes. The commit message for 5753d4ee32 claims that:
When determining whether an index update may be skipped by using
HOT, we can ignore attributes indexed only by BRIN indexes. There
are no index pointers to individual tuples in BRIN, and the page
range summary will be updated anyway as it relies on visibility
info.
This is partially incorrect - it's true BRIN indexes don't point to
individual tuples, so HOT chains are not an issue, but the visibitlity
info is not sufficient to keep the index up to date. This can easily
result in corrupted indexes, as demonstrated in the hackers thread.
This does not mean relaxing the HOT restrictions for BRIN is a lost
cause, but it needs to handle the two aspects (allowing HOT chains and
updating the page range summaries) as separate. But that requires a
major changes, and it's too late for that in the current dev cycle.

Since a117cebd6, some older gcc versions issue "variable may be used
uninitialized in this function" complaints for brin_summarize_range.
Silence that using the same coding pattern as in bt_index_check_internal;
arguably, a117cebd6 had too narrow a view of which compilers might give
trouble.

When a feature enumerates relations and runs functions associated with
all found relations, the feature's user shall not need to trust every
user having permission to create objects. BRIN-specific functionality
in autovacuum neglected to account for this, as did pg_amcheck and
CLUSTER. An attacker having permission to create non-temp objects in at
least one schema could execute arbitrary SQL functions under the
identity of the bootstrap superuser. CREATE INDEX (not a
relation-enumerating operation) and REINDEX protected themselves too
late. This change extends to the non-enumerating amcheck interface.

The only change worthy of note is that pgindent messed up the formatting
of launcher.c's struct LogicalRepWorkerId, which led me to notice that
that struct wasn't used at all anymore, so I just took it out.
The code in bringetbitmap() simply added the whole matching page range
to the TID bitmap, as determined by pages_per_range, even if some of the
pages were beyond the end of the heap. The query then might fail with
an error like this:
ERROR: could not open file "base/20176/20228.2" (target block
262144): previous segment is only 131021 blocks
In this case, the relation has 262093 pages (131072 and 131021 pages),
but we're trying to acess block 262144, i.e. first block of the 3rd
segment. At that point _mdfd_getseg() notices the preceding segment is
incomplete, and fails.
Hitting this in practice is rather unlikely, because:
* Most indexes use power-of-two ranges, so segments and page ranges
align perfectly (segment end is also a page range end).
* The table size has to be just right, with the last segment being
almost full - less than one page range from full segment, so that the
last page range actually crosses the segment boundary.
* Prefetch has to be enabled. The regular page access checks that
pages are not beyond heap end, but prefetch does not. On older
releases (before 12) the execution stops after hitting the first
non-existent page, so the prefetch distance has to be sufficient
to reach the first page in the next segment to trigger the issue.
Since 12 it's enough to just have prefetch enabled, the prefetch
distance does not matter.
Fixed by not adding non-existent pages to the TID bitmap. Backpatch
all the way back to 9.6 (BRIN indexes were introduced in 9.5, but that
release is EOL).

Commit a1c649d889 changed the signature of the BRIN consistent function
by adding a new required parameter. Treating the parameter as optional,
which would make the change backwards incompatibile, was rejected with
the justification that there are few out-of-core extensions, so it's not
worth adding making the code more complex, and it's better to deal with
that in the extension.
But after further thought, that would be rather problematic, because
pg_upgrade simply dumps catalog contents and the same version of an
extension needs to work on both PostgreSQL versions. Supporting both
variants of the consistent function (with 3 or 4 arguments) makes that
possible.
The signature is not the only thing that changed, as commit 72ccf55cb9
moved handling of IS [NOT] NULL keys from the support procedures. But
this change is backward compatible - handling the keys in exension is
unnecessary, but harmless. The consistent function will do a bit of
unnecessary work, but it should be very cheap.
This also undoes most of the changes to the existing opclasses (minmax
and inclusion), making them use the old signature again. This should
make backpatching simpler.

The bringetbitmap function allocates memory for various purposes, which
may be quite expensive, depending on the number of scan keys. Instead of
allocating them separately, allocate one bit chunk of memory an carve it
into smaller pieces as needed - all the pieces have the same lifespan,
and it saves quite a bit of CPU and memory overhead.

The handling of IS [NOT] NULL clauses is independent of an opclass, and
most of the code was exactly the same in both minmax and inclusion. So
instead move the code from support procedures to the AM.
This simplifies the code - especially the support procedures - quite a
bit, as they don't need to care about NULL values and flags at all. It
also means the IS [NOT] NULL clauses can be evaluated without invoking
the support procedure.

This commit changes how we pass scan keys to BRIN consistent function.
Instead of passing them one by one, we now pass all scan keys for a
given attribute at once. That makes the consistent function a bit more
complex, as it has to loop through the keys, but it does allow more
elaborate opclasses that can use multiple keys to eliminate ranges much
more effectively.
The existing BRIN opclasses (minmax, inclusion) don't really benefit
from this change. The primary purpose is to allow future opclases to
benefit from seeing all keys at once.
This does change the BRIN API, because the signature of the consistent
function changes (a new parameter with number of scan keys). So this
breaks existing opclasses, and will require supporting two variants of
the code for different PostgreSQL versions. We've considered supporting
two variants of the consistent, but we've decided not to do that.
Firstly, there's another patch that moves handling of NULL values from
the opclass, which means the opclasses need to be updated anyway.
Secondly, we're not aware of any out-of-core BRIN opclasses, so it does
not seem worth the extra complexity.

Add an executor aminsert() hint mechanism that informs index AMs that
the incoming index tuple (the tuple that accompanies the hint) is not
being inserted by execution of an SQL statement that logically modifies
any of the index's key columns.
The hint is received by indexes when an UPDATE takes place that does not
apply an optimization like heapam's HOT (though only for indexes where
all key columns are logically unchanged). Any index tuple that receives
the hint on insert is expected to be a duplicate of at least one
existing older version that is needed for the same logical row. Related
versions will typically be stored on the same index page, at least
within index AMs that apply the hint.
Recognizing the difference between MVCC version churn duplicates and
true logical row duplicates at the index AM level can help with cleanup
of garbage index tuples. Cleanup can intelligently target tuples that
are likely to be garbage, without wasting too many cycles on less
promising tuples/pages (index pages with little or no version churn).
This is infrastructure for an upcoming commit that will teach nbtree to
perform bottom-up index deletion. No index AM actually applies the hint
just yet.

This allows AM-specific knowledge to be applied during creation of
pg_amop and pg_amproc entries. Specifically, the AM knows better than
core code which entries to consider as required or optional. Giving
the latter entries the appropriate sort of dependency allows them to
be dropped without taking out the whole opclass or opfamily; which
is something we'd like to have to correct obsolescent entries in
extensions.
This callback also opens the door to performing AM-specific validity
checks during opclass creation, rather than hoping than an opclass
developer will remember to test with "amvalidate". For the most part
I've not actually added any such checks yet; that can happen in a
follow-on patch. (Note that we shouldn't remove any tests from
"amvalidate", as those are still needed to cross-check manually
constructed entries in the initdb data. So adding tests to
"amadjustmembers" will be somewhat duplicative, but it seems like
a good idea anyway.)

PostgreSQL provides set of template index access methods, where opclasses have
much freedom in the semantics of indexing. These index AMs are GiST, GIN,
SP-GiST and BRIN. There opclasses define representation of keys, operations on
them and supported search strategies. So, it's natural that opclasses may be
faced some tradeoffs, which require user-side decision. This commit implements
opclass parameters allowing users to set some values, which tell opclass how to
index the particular dataset.
This commit doesn't introduce new storage in system catalog. Instead it uses
pg_attribute.attoptions, which is used for table column storage options but
unused for index attributes.
In order to evade changing signature of each opclass support function, we
implement unified way to pass options to opclass support functions. Options
are set to fn_expr as the constant bytea expression. It's possible due to the
fact that opclass support functions are executed outside of expressions, so
fn_expr is unused for them.
This commit comes with some examples of opclass options usage. We parametrize
signature length in GiST. That applies to multiple opclasses: tsvector_ops,
gist__intbig_ops, gist_ltree_ops, gist__ltree_ops, gist_trgm_ops and
gist_hstore_ops. Also we parametrize maximum number of integer ranges for
gist__int_ops. However, the main future usage of this feature is expected
to be json, where users would be able to specify which way to index particular
json parts.

Introduce new fields amusemaintenanceworkmem and amparallelvacuumoptions
in IndexAmRoutine for parallel vacuum. The amusemaintenanceworkmem tells
whether a particular IndexAM uses maintenance_work_mem or not. This will
help in controlling the memory used by individual workers as otherwise,
each worker can consume memory equal to maintenance_work_mem. The
amparallelvacuumoptions tell whether a particular IndexAM participates in
a parallel vacuum and if so in which phase (bulkdelete, vacuumcleanup) of
vacuum.

Historically, the code to build relation options has been shaped the
same way in multiple code paths by using a set of datums in input with
the options parsed with a static table which is then filled with the
option values. This introduces a new common routine in reloptions.c to
do most of the legwork for the in-core code paths.

This feature was using a process local map to track the first few blocks
in the relation. The map was reset each time we get the block with enough
freespace. It was discussed that it would be better to track this map on
a per-relation basis in relcache and then invalidate the same whenever
vacuum frees up some space in the page or when FSM is created. The new
design would be better both in terms of API design and performance.

This uses the progress reporting infrastructure added by c16dc1aca5,
adding support for CREATE INDEX and CREATE INDEX CONCURRENTLY.
There are two pieces to this: one is index-AM-agnostic, and the other is
AM-specific. The latter is fairly elaborate for btrees, including
reportage for parallel index builds and the separate phases that btree
index creation uses; other index AMs, which are much simpler in their
building procedures, have simplistic reporting only, but that seems
sufficient, at least for non-concurrent builds.
The index-AM-agnostic part is fairly complete, providing insight into
the CONCURRENTLY wait phases as well as block-based progress during the
index validation table scan. (The index validation index scan requires
patching each AM, which has not been included here.)

To support building indexes over tables of different AMs, the scans to
do so need to be routed through the table AM. While moving a fair
amount of code, nearly all the changes are just moving code to below a
callback.
Currently the range based interface wouldn't make much sense for non
block based table AMs. But that seems aceptable for now.

known target block has insufficient space, we still try to insert into some
other page before giving up and extending the relation, since doing
otherwise leads to table bloat. Testing showed that trying every page
penalized performance slightly, so we compromise and try every other page.
This way, we visit at most two pages. Any pages with wasted free space
become visible at next relation extension, so we still control table bloat.
As a bonus, directly attempting one or two pages can even be faster than
consulting the FSM would have been.
Once the FSM is created for a heap we don't remove it even if somebody
deletes all the rows from the corresponding relation. We don't think it is
a useful optimization as it is quite likely that relation will again grow
to the same size.

heapam.h previously was included in a number of widely used
headers (e.g. execnodes.h, indirectly in executor.h, ...). That's
problematic on its own, as heapam.h contains a lot of low-level
details that don't need to be exposed that widely, but becomes more
problematic with the upcoming introduction of pluggable table storage
- it seems inappropriate for heapam.h to be included that widely
afterwards.
heapam.h was largely only included in other headers to get the
HeapScanDesc typedef (which was defined in heapam.h, even though
HeapScanDescData is defined in relscan.h). The better solution here
seems to be to just use the underlying struct (forward declared where
necessary). Similar for BulkInsertState.
Another problem was that LockTupleMode was used in executor.h - parts
of the file tried to cope without heapam.h, but due to the fact that
it indirectly included it, several subsequent violations of that goal
were not not noticed. We could just reuse the approach of declaring
parameters as int, but it seems nicer to move LockTupleMode to
lockoptions.h - that's not a perfect location, but also doesn't seem
bad.
As a number of files relied on implicitly included heapam.h, a
significant number of files grew an explicit include. It's quite
probably that a few external projects will need to do the same.

This patch introduces INCLUDE clause to index definition. This clause
specifies a list of columns which will be included as a non-key part in
the index. The INCLUDE columns exist solely to allow more queries to
benefit from index-only scans. Also, such columns don't need to have
appropriate operator classes. Expressions are not supported as INCLUDE
columns since they cannot be used in index-only scans.
Index access methods supporting INCLUDE are indicated by amcaninclude flag
in IndexAmRoutine. For now, only B-tree indexes support INCLUDE clause.
In B-tree indexes INCLUDE columns are truncated from pivot index tuples
(tuples located in non-leaf pages and high keys). Therefore, B-tree indexes
now might have variable number of attributes. This patch also provides
generic facility to support that: pivot tuples contain number of their
attributes in t_tid.ip_posid. Free 13th bit of t_info is used for indicating
that. This facility will simplify further support of index suffix truncation.
The changes of above are backward-compatible, pg_upgrade doesn't need special
handling of B-tree indexes for that.

BRIN indexes like to propagate additions of free space into the upper pages
of their free space maps as soon as the new space is known, even when it's
just on one individual index page. Previously this required calling
FreeSpaceMapVacuum, which is quite an expensive thing if the map is large.
Use the FreeSpaceMapVacuumRange function recently added by commit c79f6df75
to reduce the amount of work done for this purpose.
Fix a couple of places that neglected to do the upper-page vacuuming at all
after recording new free space. If the policy is to be that BRIN should do
that, it should do it everywhere.
Do RecordPageWithFreeSpace unconditionally in brin_page_cleanup, and do
FreeSpaceMapVacuum unconditionally in brin_vacuum_scan. Because of the
FSM's imprecise storage of free space, the old complications here seldom
bought anything, they just slowed things down. This approach also
provides a predictable path for FSM corruption to be repaired.
Remove premature RecordPageWithFreeSpace call in brin_getinsertbuffer
where it's about to return an extended page to the caller. The caller
should do that, instead, after it's inserted its new tuple. Fix the
one caller that forgot to do so.
Simplify logic in brin_doupdate's same-page-update case by postponing
brin_initialize_empty_new_buffer to after the critical section; I see
little point in doing it before.
Avoid repeat calls of RelationGetNumberOfBlocks in brin_vacuum_scan.
Avoid duplicate BufferGetBlockNumber and BufferGetPage calls in
a couple of places where we already had the right values.
Move a BRIN_elog debug logging call out of a critical section; that's
pretty unsafe and I don't think it buys us anything to not wait till
after the critical section.
Move the "*extended = false" step in brin_getinsertbuffer into the
routine's main loop. There's no actual bug there, since the loop can't
iterate with *extended still true, but it doesn't seem very future-proof
as coded; and it's certainly not documented as a loop invariant.

Autovacuum's 'workitem' request queue is of limited size, so requests
can fail if they arrive more quickly than autovacuum can process them.
Emit a log message when this happens, to provide better visibility of
this.
Backpatch to 10. While this represents an API change for
AutoVacuumRequestWork, that function is not yet prepared to deal with
external modules calling it, so there doesn't seem to be any risk (other
than log spam, that is.)

To make this work, tuplesort.c and logtape.c must also support
parallelism, so this patch adds that infrastructure and then applies
it to the particular case of parallel btree index builds. Testing
to date shows that this can often be 2-3x faster than a serial
index build.
The model for deciding how many workers to use is fairly primitive
at present, but it's better than not having the feature. We can
refine it as we get more experience.

AclObjectKind was basically just another enumeration for object types,
and we already have a preferred one for that. It's only used in
aclcheck_error. By using ObjectType instead, we can also give some more
precise error messages, for example "index" instead of "relation".
