This replaces dblink's blocking libpq calls, allowing cancellation and
allowing DROP DATABASE (of a database not involved in the query). Apart
from explicit dblink_cancel_query() calls, dblink still doesn't cancel
the remote side. The replacement for the blocking calls consists of
new, general-purpose query execution wrappers in the libpqsrv facility.
Out-of-tree extensions should adopt these. Use them in postgres_fdw,
replacing a local implementation from which the libpqsrv implementation
derives. This is a bug fix for dblink. Code inspection identified the
bug at least thirteen years ago, but user complaints have not appeared.
Hence, no back-patch for now.

It's not necessary to fill the key field in most cases, since
hash_search has already done that. Some existing call sites have an
assert or comment that this contract has been fulfilled, but those
are quite old and that practice seems unnecessary here.
While at it, remove a nearby redundant assignment that a smart compiler
will elide anyway.

The following changes are done:
- Addition of WaitEventBufferPin and WaitEventExtension, that hold a
list of wait events related to each category.
- Addition of two functions that encapsulate the list of wait events for
each category.
- Rename BUFFER_PIN to BUFFERPIN (only this wait event class used an
underscore, requiring a specific rule in the automation script).
These changes make a bit easier the automatic generation of all the code
and documentation related to wait events, as all the wait event
categories are now controlled by consistent structures and functions.

Run pgindent, pgperltidy, and reformat-dat-files.
This set of diffs is a bit larger than typical. We've updated to
pg_bsd_indent 2.1.2, which properly indents variable declarations that
have multi-line initialization expressions (the continuation lines are
now indented one tab stop). We've also updated to perltidy version
20230309 and changed some of its settings, which reduces its desire to
add whitespace to lines to make assignments etc. line up. Going
forward, that should make for fewer random-seeming changes to existing
code.

This reverts commit 3d03b24c3 (Revert Add support for Kerberos
credential delegation) which was committed on the grounds of concern
about portability, but on further review and discussion, it's clear that
we are better off explicitly requiring MIT Kerberos as that appears to
be the only GSSAPI library currently that's under proper maintenance
and ongoing development. The API used for storing credentials was added
to MIT Kerberos over a decade ago while for the other libraries which
appear to be mainly based on Heimdal, which exists explicitly to be a
re-implementation of MIT Kerberos, the API never made it to a released
version (even though it was added to the Heimdal git repo over 5 years
ago..).
This post-feature-freeze change was approved by the RMT.

Support GSSAPI/Kerberos credentials being delegated to the server by a
client. With this, a user authenticating to PostgreSQL using Kerberos
(GSSAPI) credentials can choose to delegate their credentials to the
PostgreSQL server (which can choose to accept them, or not), allowing
the server to then use those delegated credentials to connect to
another service, such as with postgres_fdw or dblink or theoretically
any other service which is able to be authenticated using Kerberos.
Both postgres_fdw and dblink are changed to allow non-superuser
password-less connections but only when GSSAPI credentials have been
delegated to the server by the client and GSSAPI is used to
authenticate to the remote system.

Until now dblink and postgres_fdw did not process interrupts during connection
establishment. Besides preventing query cancellations etc, this can lead to
undetected deadlocks, as global barriers are not processed.
These aforementioned undetected deadlocks are the reason for the spate of CI
test failures in the FreeBSD 'test_running' step.
Fix the bug by using the helper from libpq-be-fe-helpers.h, introduced in a
prior commit. Besides fixing the bug, this also removes duplicated code
around reserving file descriptors.
As the change is relatively large and there are no field reports of the
problem, don't backpatch for now.

Instead of dozens of mostly-duplicate pg_foo_aclcheck() functions,
write one common function object_aclcheck() that can handle almost all
of them. We already have all the information we need, such as which
system catalog corresponds to which catalog table and which column is
the ACL column.
There are a few pg_foo_aclcheck() that don't work via the generic
function and have special APIs, so those stay as is.
I also changed most pg_foo_aclmask() functions to static functions,
since they are not used outside of aclchk.c.

Per discussion, the existing routine name able to initialize a SRF
function with materialize mode is unpopular, so rename it. Equally, the
flags of this function are renamed, as of:
- SRF_SINGLE_USE_EXPECTED -> MAT_SRF_USE_EXPECTED_DESC
- SRF_SINGLE_BLESS -> MAT_SRF_BLESS
The previous function and flags introduced in 9e98583 are kept around
for compatibility purposes, so as any extension code already compiled
with v15 continues to work as-is. The declarations introduced here for
compatibility will be removed from HEAD in a follow-up commit.
The new names have been suggested by Andres Freund and Melanie
Plageman.

Since these macros just cast whatever you give them to the designated
output type, and many normal uses also cast the output type further, a
number of incorrect uses go undiscovered. The fixes in this patch
have been discovered by changing these macros to inline functions,
which is the subject of a future patch.

9e98583 introduced a helper to centralize building their needed state
(tuplestore, tuple descriptors, etc.), checking for any errors. This
commit updates all places of contrib/ that can be switched to use
SetSingleFuncCall() as a drop-in replacement, resulting in the removal
of a lot of boilerplate code in all the modules updated by this commit.
Per analysis, some places remain as they are:
- pg_logdir_ls() in adminpack/ uses historically TYPEFUNC_RECORD as
return type, and I suspect that changing it may cause issues at run-time
with some of its past versions, down to 1.0.
- dblink/ uses a wrapper function doing exactly the work of
SetSingleFuncCall(). Here the switch should be possible, but rather
invasive so it does not seem the extra backpatch maintenance cost.
- tablefunc/, similarly, uses multiple helper functions with portions of
SetSingleFuncCall() spread across the code paths of this module.

This routine is a no-op since dd04e95 from 2003, with a macro kept
around for compatibility purposes. This has led to the same code
patterns being copy-pasted around for no effect, sometimes in confusing
ways like in pg_logical_slot_get_changes_guts() from logical.c where the
code was actually incorrect.
This issue has been discussed on two different threads recently, so
rather than living with this legacy, remove any uses of this routine in
the C code to simplify things. The compatibility macro is kept to avoid
breaking any out-of-core modules that depend on it.

pgfdw_report_error() in postgres_fdw gets a message from PGresult or
PGconn to report an error received from a remote server. Previously
if it could get a message from neither of them, it reported empty
message unexpectedly. The cause of this issue was that pgfdw_report_error()
didn't handle properly the case where no message could be obtained
and its local variable message_primary was set to '\0'.
This commit improves pgfdw_report_error() so that it reports the message
"could not obtain ..." when it gets no message and message_primary
is set to '\0'. This is the same behavior as when message_primary is NULL.
dblink_res_error() in dblink has the same issue, so this commit also
improves it in the same way.
Back-patch to all supported branches.

The foreign data wrapper's validator function provides a HINT message with
list of valid options for the object specified in CREATE or ALTER command,
when the option given in the command is invalid. Previously
postgresql_fdw_validator() and the validator functions for postgres_fdw and
dblink_fdw worked in that way even there were no valid options in the object,
which could lead to the HINT message with empty list (because there were
no valid options). For example, ALTER FOREIGN DATA WRAPPER postgres_fdw
OPTIONS (format 'csv') reported the following ERROR and HINT messages.
This behavior was confusing.

HINT: Valid options in this context are:
There is no such issue in file_fdw. The validator function for file_fdw
reports the HINT message "There are no valid options in this context."
instead in that case.
This commit improves postgresql_fdw_validator() and the validator functions
for postgres_fdw and dblink_fdw so that they do likewise. For example,
this change causes the above ALTER FOREIGN DATA WRAPPER command to
report the following messages.
ERROR: invalid option "nonexistent"
HINT: There are no valid options in this context.

Invent a new flag bit HASH_STRINGS to specify C-string hashing, which
was formerly the default; and add assertions insisting that exactly
one of the bits HASH_STRINGS, HASH_BLOBS, and HASH_FUNCTION be set.
This is in hopes of preventing recurrences of the type of oversight
fixed in commit a1b8aa1e4 (i.e., mistakenly omitting HASH_BLOBS).
Also, when HASH_STRINGS is specified, insist that the keysize be
more than 8 bytes. This is a heuristic, but it should catch
accidental use of HASH_STRINGS for integer or pointer keys.
(Nearly all existing use-cases set the keysize to NAMEDATALEN or
more, so there's little reason to think this restriction should
be problematic.)
Tweak hash_create() to insist that the HASH_ELEM flag be set, and
remove the defaults it had for keysize and entrysize. Since those
defaults were undocumented and basically useless, no callers
omitted HASH_ELEM anyway.
Also, remove memset's zeroing the HASHCTL parameter struct from
those callers that had one. This has never been really necessary,
and while it wasn't a bad coding convention it was confusing that
some callers did it and some did not. We might as well save a few
cycles by standardizing on "not".
Also improve the documentation for hash_create().
In passing, improve reinit.c's usage of a hash table by storing
the key as a binary Oid rather than a string; and, since that's
a temporary hash table, allocate it in CurrentMemoryContext for
neatness.

Move the system catalog index declarations from catalog/indexing.h to
the respective parent tables' catalog/pg_*.h files. The original
reason for having it split was that the old genbki system produced the
output in the order of the catalog files it read, so all the indexing
stuff needed to come separately. But this is no longer the case, and
keeping it together makes more sense.

The comments in fd.c have long claimed that all file allocations should
go through that module, but in reality that's not always practical.
fd.c doesn't supply APIs for invoking some FD-producing syscalls like
pipe() or epoll_create(); and the APIs it does supply for non-virtual
FDs are mostly insistent on releasing those FDs at transaction end;
and in some cases the actual open() call is in code that can't be made
to use fd.c, such as libpq.
This has led to a situation where, in a modern server, there are likely
to be seven or so long-lived FDs per backend process that are not known
to fd.c. Since NUM_RESERVED_FDS is only 10, that meant we had *very*
few spare FDs if max_files_per_process is >= the system ulimit and
fd.c had opened all the files it thought it safely could. The
contrib/postgres_fdw regression test, in particular, could easily be
made to fall over by running it under a restrictive ulimit.
To improve matters, invent functions Acquire/Reserve/ReleaseExternalFD
that allow outside callers to tell fd.c that they have or want to allocate
a FD that's not directly managed by fd.c. Add calls to track all the
fixed FDs in a standard backend session, so that we are honestly
guaranteeing that NUM_RESERVED_FDS FDs remain unused below the EMFILE
limit in a backend's idle state. The coding rules for these functions say
that there's no need to call them in code that just allocates one FD over
a fairly short interval; we can dip into NUM_RESERVED_FDS for such cases.
That means that there aren't all that many places where we need to worry.
But postgres_fdw and dblink must use this facility to account for
long-lived FDs consumed by libpq connections. There may be other places
where it's worth doing such accounting, too, but this seems like enough
to solve the immediate problem.
Internally to fd.c, "external" FDs are limited to max_safe_fds/3 FDs.
(Callers can choose to ignore this limit, but of course it's unwise
to do so except for fixed file allocations.) I also reduced the limit
on "allocated" files to max_safe_fds/3 FDs (it had been max_safe_fds/2).
Conceivably a smarter rule could be used here --- but in practice,
on reasonable systems, max_safe_fds should be large enough that this
isn't much of an issue, so KISS for now. To avoid possible regression
in the number of external or allocated files that can be opened,
increase FD_MINFREE and the lower limit on max_files_per_process a
little bit; we now insist that the effective "ulimit -n" be at least 64.
This seems like pretty clearly a bug fix, but in view of the lack of
field complaints, I'll refrain from risking a back-patch.

We used to strategically place newlines after some function call left
parentheses to make pgindent move the argument list a few chars to the
left, so that the whole line would fit under 80 chars. However,
pgindent no longer does that, so the newlines just made the code
vertically longer for no reason. Remove those newlines, and reflow some
of those lines for some extra naturality.

The basic rule we follow here is to always first include 'postgres.h' or
'postgres_fe.h' whichever is applicable, then system header includes and
then Postgres header includes. In this, we also follow that all the
Postgres header includes are in order based on their ASCII value. We
generally follow these rules, but the code has deviated in many places.
This commit makes it consistent just for contrib modules. The later
commits will enforce similar rules in other parts of code.

heapam.h previously was included in a number of widely used
headers (e.g. execnodes.h, indirectly in executor.h, ...). That's
problematic on its own, as heapam.h contains a lot of low-level
details that don't need to be exposed that widely, but becomes more
problematic with the upcoming introduction of pluggable table storage
- it seems inappropriate for heapam.h to be included that widely
afterwards.
heapam.h was largely only included in other headers to get the
HeapScanDesc typedef (which was defined in heapam.h, even though
HeapScanDescData is defined in relscan.h). The better solution here
seems to be to just use the underlying struct (forward declared where
necessary). Similar for BulkInsertState.
Another problem was that LockTupleMode was used in executor.h - parts
of the file tried to cope without heapam.h, but due to the fact that
it indirectly included it, several subsequent violations of that goal
were not not noticed. We could just reuse the approach of declaring
parameters as int, but it seems nicer to move LockTupleMode to
lockoptions.h - that's not a perfect location, but also doesn't seem
bad.
As a number of files relied on implicitly included heapam.h, a
significant number of files grew an explicit include. It's quite
probably that a few external projects will need to do the same.

Previously tables declared WITH OIDS, including a significant fraction
of the catalog tables, stored the oid column not as a normal column,
but as part of the tuple header.
This special column was not shown by default, which was somewhat odd,
as it's often (consider e.g. pg_class.oid) one of the more important
parts of a row. Neither pg_dump nor COPY included the contents of the
oid column by default.
The fact that the oid column was not an ordinary column necessitated a
significant amount of special case code to support oid columns. That
already was painful for the existing, but upcoming work aiming to make
table storage pluggable, would have required expanding and duplicating
that "specialness" significantly.
WITH OIDS has been deprecated since 2005 (commit ff02d0a05280e0).
Remove it.
Removing includes:
- CREATE TABLE and ALTER TABLE syntax for declaring the table to be
WITH OIDS has been removed (WITH (oids[ = true]) will error out)
- pg_dump does not support dumping tables declared WITH OIDS and will
issue a warning when dumping one (and ignore the oid column).
- restoring an pg_dump archive with pg_restore will warn when
restoring a table with oid contents (and ignore the oid column)
- COPY will refuse to load binary dump that includes oids.
- pg_upgrade will error out when encountering tables declared WITH
OIDS, they have to be altered to remove the oid column first.
- Functionality to access the oid of the last inserted row (like
plpgsql's RESULT_OID, spi's SPI_lastoid, ...) has been removed.
The syntax for declaring a table WITHOUT OIDS (or WITH (oids = false)
for CREATE TABLE) is still supported. While that requires a bit of
support code, it seems unnecessary to break applications / dumps that
do not use oids, and are explicit about not using them.
The biggest user of WITH OID columns was postgres' catalog. This
commit changes all 'magic' oid columns to be columns that are normally
declared and stored. To reduce unnecessary query breakage all the
newly added columns are still named 'oid', even if a table's column
naming scheme would indicate 'reloid' or such. This obviously
requires adapting a lot code, mostly replacing oid access via
HeapTupleGetOid() with access to the underlying Form_pg_*->oid column.
The bootstrap process now assigns oids for all oid columns in
genbki.pl that do not have an explicit value (starting at the largest
oid previously used), only oids assigned later by oids will be above
FirstBootstrapObjectId. As the oid column now is a normal column the
special bootstrap syntax for oids has been removed.
Oids are not automatically assigned during insertion anymore, all
backend code explicitly assigns oids with GetNewOidWithIndex(). For
the rare case that insertions into the catalog via SQL are called for
the new pg_nextoid() function can be used (which only works on catalog
tables).
The fact that oid columns on system tables are now normal columns
means that they will be included in the set of columns expanded
by * (i.e. SELECT * FROM pg_class will now include the table's oid,
previously it did not). It'd not technically be hard to hide oid
column by default, but that'd mean confusing behavior would either
have to be carried forward forever, or it'd cause breakage down the
line.
While it's not unlikely that further adjustments are needed, the
scope/invasiveness of the patch makes it worthwhile to get merge this
now. It's painful to maintain externally, too complicated to commit
after the code code freeze, and a dependency of a number of other
patches.

This patch introduces INCLUDE clause to index definition. This clause
specifies a list of columns which will be included as a non-key part in
the index. The INCLUDE columns exist solely to allow more queries to
benefit from index-only scans. Also, such columns don't need to have
appropriate operator classes. Expressions are not supported as INCLUDE
columns since they cannot be used in index-only scans.
Index access methods supporting INCLUDE are indicated by amcaninclude flag
in IndexAmRoutine. For now, only B-tree indexes support INCLUDE clause.
In B-tree indexes INCLUDE columns are truncated from pivot index tuples
(tuples located in non-leaf pages and high keys). Therefore, B-tree indexes
now might have variable number of attributes. This patch also provides
generic facility to support that: pivot tuples contain number of their
attributes in t_tid.ip_posid. Free 13th bit of t_info is used for indicating
that. This facility will simplify further support of index suffix truncation.
The changes of above are backward-compatible, pg_upgrade doesn't need special
handling of B-tree indexes for that.

Per the project style guide, details and hints should have leading
capitalization and end with a period. On the other hand, errcontext should
not be capitalized and should not end with a period. To support well
formatted error contexts in dblink, extend dblink_res_error() to take a
format+arguments rather than a hardcoded string.

AclObjectKind was basically just another enumeration for object types,
and we already have a preferred one for that. It's only used in
aclcheck_error. By using ObjectType instead, we can also give some more

The lower case spellings are C and C++ standard and are used in most
parts of the PostgreSQL sources. The upper case spellings are only used
in some files/modules. So standardize on the standard spellings.
The APIs for ICU, Perl, and Windows define their own TRUE and FALSE, so
those are left as is when using those APIs.
In code comments, we use the lower-case spelling for the C concepts and
keep the upper-case spelling for the SQL concepts.
